Complaint against Care One at Wall, 2621 Highway 138, Wall NJ 07719
Sorry for delay of complaint was waiting to get my mother, G. S. home with live-in.

My mother went to Jersey Shore Hospital on July 25, diagnosis pace maker.
Due to the fact she was on Coumadin, surgery was delayed until August 3, when her INR level was consistent for surgery.
She was released on August 4, late afternoon to Care One in wall. I looked at two facilities and choose them due to some of her friends that were there over a year ago.
I visit my mom almost everyday, if not me a friend of the family Barbara did.
First thing, when they did the admission I was not allowed in the room, mind you at the time my moms mental state was a little confused so her answering her own questions is a little scary. For example, I stayed with her in the hospital everyday because she would answer the doctors incorrectly about her history and how she felt.
When she left the hospital on the 4th, her ankles were normal (normal for her she has always had a little swelling and takes Lasik.) I noticed on the 5th that her lower legs were getting a little more swollen, and they assured me she was receiving Lasik. Everyday I went it got worse, to a point that I called her Doctor on August 9, 2011 because I was concerned her legs (both) were enormous, like elephant legs, again they assured me at Care One that she was fine and that she was receiving her Lasik. My husband and I even spoke in person with the administrator George Mervine, we were discussing the worry regarding her legs and the fact that when she was sitting in PT for 2hours, she only rec’d about ½ hour of attention, so why be there at all! (In regards to that, my mom was able to do a little walking in hospital with walker, but none at all at CareOne, Im sure the swelling of the legs didn’t help.) George said he would have the PT and Nurse contact me, that they have there meetings and they would call me the next day. They never did by the way.
So after I called my moms doctor he called me on the 10th and told me to go there and tell them that he said she needs to go back to Jersey Shore due to the enormous ( and im not exaggerating, I have taken care of my mom for along time this was crazy) swelling in the legs. I left work and went there and told the head nurse, sorry I don’t have her name, but it was on August 10 if that helps. She told me not to worry my mother was fine and she was getting the best care where she was. I said really, I have been complaining since the second day that her legs were hugh, the swelling is not normal and no nurse has addressed this or shown concern. She insisted she could stay and was fine, after arguing with her and saying that I, her daughter, and her doctor want her to go to Jersey Shore; she finally went to find her. She was in PT, there she listened to my moms lungs and told me they sounded fine and her vitals where fine, I told her I don’t care she is going to the hospital.
Long story short, got her out of rehab to Jersey Shore ER, she was in congestive heart failure, water in the lungs (not too much but a concern) and later found out more blood clots. So really! Head nurse said she was fine, lungs where fine!
I feel, deep down inside I know, if I would have left my mother there she would not have gotten out alive. She ended up in hospital from the 10 till the 16. Then went to another rehab facitly that was amazing! The care at Meridian of Shrewsbury was wonderful. This would be the third rehab facility for my mom in the past two years, and the staff and everyone working there honestly do care, they are fantastic. They always answered all my questions, doctor called me back if I was concerned, and they never left the people that could not fend for themselves sitting in the hallways.
And before I picked Shrewsbury I did my research, and checked other facilies, I was not going to make a mistake with my mom’s life again. They were highly recommended and when visiting other facilities I met ex-employees of Care One at Wall and was told the minute the new administration came on, I guess a year ago, that Care One went down hill. There were two that worked at Care One and both said it used to be a wonderful caring place but not anymore. And it was evident when my mom was there; I saw how they would just leave elderly people in the halls, especially if no one visited them. And they would ask for assistance and the people would just walk by, and no they weren’t doing anything I watched…and saw other visitors helping the people that couldn’t help themselves. And look what happened to my mother. I feel that Care One is more about the money they receive.

I heard from other families that had people there that they were keeping records of what was going on because they too were concerned about their loved one.

I thought that it was very important to bring this facility to your attention, Care one in Wall should be called Care None.

I agree! I worked as a employee at this facility. I HAVE SEEN THINGS GO ON IN THIS FACILITY THAT YOU WOULD NEVER BELIEVE. One of which being that the director of nursing has her mother in law there as a resident. She hates here mother inlaw so anytime she falls or is in pain she tells the staff not to document the incident and not to give her pain meds! sHe makes them lie about giving her meds that she told them not to give her.She threatens people jobs left and riGht and also talks to employees In a degrading way. Humiliating them in front of families .Dont take your family their they just want your money.

My mother is currently in CareOne and it is the biggest mistake we ever made. Fortunately, she is being discharged today.
Care and compassion are not what this place is about, it's only about money. Don't ask for anything special from the kitchen staff, even filling out the menu is a joke. They will give you what they want to give you, including an empty plate with a cover on it.
The place is filthy so I wonder how they can safely care for post surgical patients with open wounds. They don't seem to care, the floors are mopped with gray, dirty water and mops that are the same.
And yes, PT is a joke. The only people that are doing well are the ones young enough to be able to push themselves, the older patients are left for hours in a wheel chair until they can "get to them." Hell, even the wheel chairs are filthy.
Do not expect someone to come when you use the Call Nurse button, it can and has taken upwards of an hour to get a response.
If you expect your family member is not receiving their proper medication, go with your gut instinct. We found pills on the floor in my mother's room that were NOT meds prescribed to her. When I brought it to the nurse's attention, I got a shrug of the shoulders.
The photo below is a sample of what came off the floor in my mother's room right after it was mopped.

My mother stayed at Care-One for one month under hospice care. The staff was so inattentive that I had to hire additional care givers to come in to take care of her because I could not rely on the staff at Care-One. Often times I would walk in to find the phone out of reach and the call button on the floor. The staff rarely, if ever, tried to engage my mother in conversation or even refer to her by name. The water my mother was given was from the tap. Her dietary requests were rarely met and even when they were the food was inedible. While all of this is horrible, it pales in comparison to the lack of air conditioning on the second floor. In the summer months the air conditioning is non existent. Three days before my mother passed away the temperature in her room was 85 degrees. It was inhuman to treat someone that way. While I was a strong advocate for my mother, many at the facility have no one to vocalize their issues and they are left to scream for help in the halls. In most cases, their screams are ignored. I'm surprised Care-One has not been shut down by the Board of Health or sued by one of their patients. Care-One made what was the most difficult time in my life and my mother's life even worse. If I felt my mother could have been successfully moved, I would have moved her to another facility within the first week. The management should hang their heads in shame treating the elderly in such inhuman conditions.

My husband went to Care One after a successful knee replacement in December. The nurse did not do an valuation as they had an emergency. That was understood. During eval 2 hrs after arrival we explained he had a bypass 24, years ago and was a diabetic. For a few days in the hospital he was on insulin to stabilize blood sugars and this was explained to the nurse. Upon discharge from the hospital his sugars were stable. I went to visit the next day and by evening his blood sugars were very high which I am sure was due to not eating properly. I requested they administer insulin but was advised they needed the medical director tonauthorize and they would call a sap. Went back next day at noon and blood sugars higher and no insulin was given . When questioned I was advised medical director was on vacation and his office neglected to let the covering dr know. Unbelievable. Finally insulin was given at night but a whole 24 hours after it was needed. We were told a dr would be in to see him but no dr ever came. On the 4 the day after rehab he was very short of breath. Went to nurse station and finally someone came down with the respiratory therapist to administer oxygen. They said an X-ray would be taken of the knee to rule out a blood clot but they do not have a portable eras and it would take an hour. Unacceptable. After another hour the breathing was awful to the point he could not talk. Requested an ambulance to take him back to he hospital. Was told it would take an hour. After a few minutes and berating was even worse ran back to the nurse station and said I want one now. They were going to check on time arrival and I said I am calling 911 if you don't call for paramedics who arrived in 15 minutes. At the hospital it was determined he had a heart attack and was in heart failure. I would not send my dog to Care One as they should be called Care None. Why they did not evaluate him properly I'll never know. Just total terrible care in an emergency. If that was a person alone I believe they would die during the night. Also, not one nurse wore a name tag with credentials so were they an RN or LPN? Not very professional. Stay aware from Care One in Wall.

For all that has lost a loved one... I had just lost my mom with alzheimers. she laid all weekend not being able to swallow with no nurses looking in on her on monday at 545 in the morning I get a call that she cannot swallow and that i should get my affairs in order.. i called hospice right away and they went and said she has hours to live. her grandson ran over but it was to late and he ran in and saw her dead. I want to bring a class action suit blaming care one for the pain and suffering they have cause upon many grieving relatives that put their loved one in the care of this uncaring facility that only cares for the money and not the care that they claim they give to our loved ones... I had gotten a call from a man who could not speak english when he tried to tell me my mother died and the funeral home could not understand hime either. My mom was there for 5 years with failing dementia and them alzheimers and many time she was dirty and soilled and they left her there like that ...I am outraged that she has to lay in a funeral home for 3 days now and we could not get a hold of the social worker named kaitlen for 3 days ... this is an outrage and has to be stopped. please respond so we can end this awfull end that our loved ones had to live with without our knowledge... Thank you and I hope we could meke a difference...

For all who put care one in charge of taking care of your loved one and believing they will take the best care of them. Please thing again. Let us put a stop to hurting and not taking care of them and bring a suit to stop ... respond and we will move forward with this request. and hoping that all who enter care one will be treated with love and respect for all that enter. My mother still cannot rest for three days because care one will not respond to me or the funeral home... beyond said and this has to end... thank you Lisa

Stay far far away,
My Uncle was here for 3 weeks in December 2016
They charged us 12k a month.
They also advised us to get a one on one for and additional $400 per day.
A Joke all they did was play on cell phones and steal our money...
We did as they recommended, lets see he fell out of the bed under 24 hour one on one care, was left in a dirty diaper for 10 hours plus daily, not cleaned daily, left in dirty clothes for 4 days at a time, found blood stains on his sheets several times. (Bio Hazards) due to IV.
You would think for that amount of money he would he treated great.
He was treated like a piece of trash.
He passed away there, wife was called 35 minutes after he was given morphine for the first time.
The neglected him and eventually killed him.
All this place wants is money people are just another piece of meat.

More to come.
I will expose them to the media...

Care One at Wall NJ is Checked few sub acute rehab on a Sunday afternoon. It was calm and quiet. Totally different story come go time.Dont be misled by the nice crown moldings and tile floors. Nor the guy that shows you the place that could work at any used car lot

Horrific care. Overworked and they let you know it. Hard to distribute meds and text your beu at the same time. 90% of them were bothered by any sort of request such as a clean towel or asking to clean the dirty toilet. I got my wife out in 2 days . No way in Hell was I leaving her in there for the full stay. Had I ignored the warning signs, the first 5 days would have had her sitting there with putting a dent in her mattress sucking up her medical insurance.

If you dont listen and do stay, pay attention to your meds that come late. You may mistakenly not get your pain meds. And if your loved on doesnt have an appetite, they may not eat that day. Ive seen full trays of go back. Ive sincerely seen food at homeless shelters better quality and served with more care. The only person that earned her pay and did the job of two people was the sweet woman working the front desk.

Im not one that is snooty or likes to complain for the sake of it. Im telling our story to help you make an informed decision.
