I visited monarch dental over 2 weeks ago to get a cleaning. After being there 1 hours I was told I needed a doctor release. No one called me to follow up with my requested services. I called a week ago and the office mgr said she would call didn't. I called today and same excuse and was asked to call back tomorrow. Very unprofessional and not a pleasant...

I paid $100 10 years ago for my stepson to have teeth whitening. He never had it done. I have called several times over the years asking for a refund to no avail. I became ill and forgot again about it and today was told all of a sudden that they had done the service (couldn't have as he left the State) and because it had been 10 years I would never see the money! I don't care how long it has been it is still my $100 and if they didn't refund me it should have gone to the State of Texas if you couldn't locate us. We haven't moved or changed our numbers! I had not been to dentist in quite some time and signed up with Humana c/o Monarch Dental. I had deep cleaning on both sides and upon leaving, I expressed to secretary I would be brushing & flossing daily so would not need deep cleaning for next appointment. My next appointment, I got no confirmation call *&* my appointment was for deep cleaning. How unsatisfying & unsettling.

i had an appointment in Hot Springs Arkansas with monarch dental. I arrived late because I am an oxygen patient and this causes physical disabilities. i sat down in the office to catch my breath, my son told the recept i was there. she said i was late and would have to reschedule. there were only two patients in the waiting room and the dentist was not going to perform any dental work that day. i was simply stunned. i have paid Monarch $2300 this year for my son. now his crown is not big enough, the dentist changed his appointment i never complained. i told them i would not be back and will tell everyone how they treat handicap patients. I am a loyal customer, who has utilized services for both my daughter and myself through monarch dental in cedar hill, texas.
On monday, october 29, the office manager attempted to charge me on a care credit account instead of notifying me of another possible in network dentist. I was not made aware of the other dentist even after asking him. He deliberately tried to scam me which I did not appreciate! His name is mario! I am truly pissed especially since he stated they "worked" me in and it would take 30 minutes to get in touch with my insurance company to find out if the dentist is in network. My question is why would you not know if your dentists are in certain networks if you are the office manager.
unauthorized charge on the wrong bank account.
I Vanadee Mcghee had dental work done for an implant on 8/24/18 at the Mansfield location off of Debbie Lane #106, although my original office where this plan was suppose to take place was at the Duncanville location off Cedar Ridge Dr, however, kirk the guy in charge of signing customers up for same day Implants told me to come to location #106 to get treatment done at the Mansfield location because that was the location they were going to be at and to come there. Kirk mentioned all the materials that I needed to go further with the plan including the amount for the payment. I was told to bring my daughter in along with a blank or voided check for the both of us to be able to use in house finance and that she needed to be there to sign documents because she will be the one financing with Smile Brands finance retail contract, because I was told she could finance for me to get work done if I don't have enough funds to cover, well the amount I got approved for wasn't enough when I applied but when my daughter applied the amount she was approved for covered for the whole plan and that I didn't need to use the amount I was approved for. so we just used her finance information for my plan of work. so on 9/24/18 the payment was supposed to be debited out of her account in the amount of $131.01 in which I transferred the funds to her account from my account, well smile brands debited from the wrong account which was mines. I don't understand how? when she gave them her account information. so I noticed the funds were taking out of my account, so I called them on 9/27/18 to let them know how they have the wrong information, so the right account information was collected for future payment, they also mention my account information was the only information they had on file so I decided to call the manager at monarch her name was ginger, she couldn't help me and gave me Kirk's personal cell number. when I finally got a hold to Kirk he mentioned that he couldn't provide me with an error email that I had asked for for my bank to speed the process up because at this point smile brand made my account go into the negative, so Kirk proceeded to tell me that they only have one voided check on file which was mines and never received Vantasia's at all and that he was going to investigate and give me a call back, so I ask Kirk "well don't you remember taking the information at the office and he couldn't answer my question so he proceeded to tell me that he will drive over to the Mansfield location to look into the file himself if he has to, to see what's going on. I remember that day like it was yesterday, Kirk was training a very young lady who kept on making errors that day while taking my payments. because I was never told that I would have to pay a percentage the day of so I used my bank card and credit card at the time of purchase, also the young lady yelled out your card is declining very loud in front of other customers, so I explained to her that I was on the phone with the bank and that couldn't be possible, come to find out she was putting over the amount that I was being charged for but then realized it was supposed to be a lesser amount, she kept on apologizing for her mistake and at this point I was already frustrated because of the wait of time for the procedure and the errors that was made after that. also, she and Kirk attached someone else's treatment plan that was with Vantasia contract, which is another error. the reason for my complaint was for an error letter for my bank, but it seems like they are up to something else on their end but all I wanted was for them to put my money back into my account and withdraw from my daughter Vantasia's account and pay that negative fee for debiting out of the wrong account but they are making this a bigger mess than what it really is. now I really want to cancel the whole thing and find somewhere else to get service from because they are unorganized and unprofessional.in my honest opinion I think it will only be fair to just delete the whole account.# [protected] chart #2142390 office# 30350
first visit
I was a new patient. Looking at some reviews about Dr.Ayesha Malik, I made a appointment for myself and my daughter. We filled in the paperwork and they called me in for exam and I was waiting for like half hour and they text telling that they have problem with the machine as I am a new patient, I thought they had problems setting up the info. After further wait, finally they told that they have problem with the X-Ray machine as over the weekend. My appointment was on Wednesday. Did they not see if the machines were working or not for the For the past 3 days. I thought that's fine it happens and rescheduled appointment for following Tuesday. I was 10 minutes late. Checked in and was waiting in the front Office. After like 12 to 15 minutes, called me in and asked me to sit in the room
Where the X-ray machine was. This is not where I waited the first time for X-rays. Even here I waited for 10 to 15 minutes and went out to see what's happening and then they say that the X-ray machine has problem. Don't they plan ahead. When someone from the office called me the before day, I asked them if they don't have any issues with the machine. The lady was not aware. I don't think they have proper planning. Or if they are not accepting new patients they should have not given the appointment. Please check more reviews before you want to go here. They did not even had a min curtesy to call and reschedule.
Dr. Louseau lied to his insurance company
Went to this horrible Dr. for a follow up visit for 3 implants he had done. He took off the temps that looked like my regular teeth and put on these over size temps that looked like horse teeth. This was done after he trimmed my gum, because one screw would not screw down. He was told I was going on cruise next morning, ask if it could wait to I was getting back, because my son was getting married. Said no it would just take 20 minutes and I would be out of there. Yes I got a infection, did not realize my gums had a red streak until 3rd day. Went to ship Dr. He confirmed I had a gum infection. Just found out he lied to his insurance co. He told them he told me to rinse with anabiotic rinse. He did not say a word to me about doing this, he did not write me a prescription or provide me with any. Told his insurance co. He did and they had the stuff at their office. He did not write a prescription or give me any. Recorded nothing in his notes, that I had before he tells this lie. I got very sick and now his insurance co is trying to say I could have pick this up on ship. Yes people can get sick on a cruise, but they do not get red streaks on gum where dentist did work.
I plan to report this to all tv new stations, BBB and corp office. This dr. Has a rating below 3%. The corp office should review his work on line and reviews. Not good for representing the Monarch organization. Monarch needs to look at this Dr. Louseau
I also plan to write a review every month to I get a letter from Monarch.
Now I have 3 teeth he did that does not match my other teeth at all . Implants are twice the length and twice as thick has my other teeth. My regular dentist for years at Monarch has been wonderful but Dr. Louseau should be checked out. Do not go to Dr. Louseau Very very bad. It one thing to make a mistake and not give me an anabiotic or a mouth rinse but to lie to his insurance co. when the patient had a real issue is a problem.

I received a call on May 29th stating one of my Family members had a substantial credit. I was advised a check would be mailed. This was from a service that occurred over 2 years ago. By June 28th I still had not received the check so I called to find out what was going on. I was told the check had not been cut and one was being requested. I was also told it would be an additional 4 to 6 weeks. In addition there was a 2nd credit now owed me as well. These issues are in addition to problems with how much I would owe for something my insurance said was covered 100%. I can guarantee I will be finding a new provider at this time.
neglect/rude
I went to get a tooth abstract February 2018 at Monarch Cedar Hill tx. I lost antibiotics before prescription was completed. I called the office and they set up an appointment to get a prescription refilled. I went in to a new Dentist I never saw before . She looked at my x rays and had me to open my mouth and started back tracking everything my last doctor that did the removal of my tooth Feb 2018 saying why does she has the labeled as (some code number) this is not a (code number) and start digging in my mouth while she asking me rudely why do you have so much plaque on your teeth. Then start digging in a tooth that did not hurt and last appointment only needed a crown. So I have been calling and complaining but respectfully about the situation and office manager Mario was very chalant and rude already having his mind set to it was my fault because my teeth were not were they were supposed to be, but if the dentist never dug in it when I was just there for a prescription refill I would of made it to get my crown. I was told that there was nothing that they could do and and I knew my teeth were bad when I came

I had 2 crowns done on bottom both back molars.. One of which they permanently cemented the temporary and I had to wait for it to break to be fixed.. On e this happened I was left with jagged edges slicing my tongue all to pieces until monday at which time the ‘dentist' accused me of biting her (lol.. I think she didn't even know why I was there)
But she stuck her hand in my mouth and was stabbed by the jagged tooth I dealt with all weekend.
Got that fixed in all her haste and hatefulness... Tried to ask her about some mild pain I was having on the other side.. Was told I had some recession and sensitivity and to get sensodyne.. No xray!
To return a month later with horrible pain on that some other side.. X rays finally offered.. Was told I had gum recession so it was sensitive and I might need a crown in a year or so on that tooth as well because it looks like there might be a crack under that filling.. Went back the next day asking for a sealant I had heard about to seal off the sensitivity.. Did not work.. More x rays to be told she saw nothing going on.. Next day I couldn't take the pain any longer.. Walked into a different dental office begging for relief.. X rays told of a horrible infection and completely broken on all 4 sides tooth! Tried to do a root canal.. Ultimately had to have the extracted because there was no saving it at this point.
Now the crown on that other side is hurting.
Someone from monarch should call me soon with a plan to make me happier.
wrong teeth removed
On the 23rd of May I visited the American Fork, Utah, location to have 3 teeth extracted by Dr Steele. Before the procedure we discussed the removal of the 2 front teeth which were broken and cracked and a molar on the bottom right side. It took about 30 minutes for him to locate a vein, which at one time he dozed off and one of the assistance gently nudged him. After surgery I returned home and slept some. After waking up I was in pain and realized ask the pain was towards the upper and lower right side of my mouth. That's when I noticed the 2 front teeth were still there but the upper back teeth which were good were gone. I called the office but he and his assistants were what gone for the day. The next day I called the South Temple location and talked with his assistants. I was able to go in and have the 2 bad teeth removed. But now I'm without 2 good teeth. Also, he wrote a prescription for an antibiotic, which after asking what I was allergic to, the pharmacist informed me what was prescribed I was also allergic to as well. Is Monarch Dental going to rectify this and replace the teeth wrongly removed so I'm not without? I'm not one to complain, but this was a horrible mistake made by the oral surgeon that should be fixed. I've been told to obtain a lawyer, but thought I'd voice my complaint directly to monarch first. Please let me know how this can be rectified. Thank you.

implant dentures
I had full upper/lower implant dentures done by Monarch Dental, Golden Triangle Mall, Denton, Texas. I paid in full $37, 000.00 for this work. The implants have failed after little more than one year. I have moved out of Texas during my care and was referred to an Ohio Monarch Dental location. This office did not do implants. They, in turn, referred me to a dentist in Gahanna. I have been struggling since last June 2017 to get some relief to these ill-fitting implants. Most recently, the best efforts of these dentist were unsuccessful and I was referred to a prosthodontics specialist who declared that he had never seen such dentistry and that the work did not comply with any dental standard of care. He pronounced my implants as FAILED. Over the last 9 months my communications with the district office of North Texas for Monarch Dental have produced only rare success. Now, as of today, they do not even return my calls or texts. I am at my wits end as to what to do. I wish there were a case action suit against Monarch Dental/SmileBrands Dentistry. This treatment has been abominable. Please let me know if there is anything I can do.

Do not, I repeat Do not go to the Monarch Dental in the golden triangle mall! I walked in last week and the front staff did not even acknowledge me, I had to ask if there was anything I needed to fill out. They called me back to the room and it was soooo gross. The light fixture that the dentist uses was filthy and the floors were so brown. I thought it would be a fast and easy tooth extraction that despite my bad feeling of getting up and walking out, I decided to stay. I do not even know the Dr's name, she was very rude when I told her that I wanted it pulled out instead of an expensive root canal!
It took her 2, 2 hours for her to pull it out, she mutilated my gums and part of the gums that hold the other tooth in is not even connected anymore. Now after the infection is going away, there's either part of the tooth or my bone that is uncovered and sticking out! I will now have to go to another place and see what has to be done to fix the issue and because of that dentist I am now scared to death again.
stripped screw on dental implant
I paid (in full)for 2 dental implants. Today (11/9/2017), for the 2nd time I went to a different office to get them placed only to learn that one of the screws had been 'stripped' and implant could not be placed. This has been going on since 2/2017. Now what? I was informed that removing the implant was a difficult process. I was a professional for 25 years, now retired. I believe in accountability when things go awry from the intended outcome. The dentist at the Seminary office was busy with a phone call while the technician was tasked with placing the implant. When he alerted the dentist of the 'screw problem' the dentist then tried to resolve it. At that point the screw was already stripped, the process to place the implant failed. After what was to be a 30 minute visit to get implants placed, I left, in tears, after 3 hours of manipulating screw into the site...with no pain management of any kind. I am upset. No matter how this is resolved I will seek a second opinion because I no longer have confidence in the competency of Monarch Dental, specifically the negligent behavior of the 'dentist' whose call was more important that a client, whether the cost was $4 or $4000. Wanna be done but 'caught up' in this process so stuck until implants are in. Wow, tremendously disappointed and scared problems related to the 'stripped screw' will affect my future health.
Called Monarch Cooperate Office when I arrived home. Within 48h they say I will be contacted. Just 'Wow'
potential crown work
Went to Dr. Patel at Monarch Dental, October, 2017. Xrays of a tooth that I was concerned about were taken. I had gone to another local dentist, out of my network for a cleaning and he suggested I have a crown put on this specific tooth because he thought it would potentially crack.
Xrays were taken at Monarch of this tooth and Dr. Patel read the xray.
She said that I had two options: do the crown work without a root canal, with the possibility of tooth sensitivity later in the tooth, OR, have a root canal done before the crown .
I asked the prices of both procedures, which the office printed out using the Cigna dental insurance that I had.
I opted to just have the crown work done and made an appointment for
November 4, this past Saturday.
Arriving at the appointment for the scheduled CROWN ONLY work, I was told by the receptionist that I HAD to have a root canal first . That is NOT what Dr. Patel had told me. She went to speak with Dr. Patel, who NEVER even had the courtesy to speak with me personally. I understood completely what Dr. Patel had told me after I left Monarch Dental the first time and the option that I had selected.
I am SO DISGUSTED at how I was treated ! I would like a response.

Receptionist ignored us when we came in. Filled out registration pad and stood waiting. Finally she said she would be with us in a minute and continues talking to coworker about personal items. Totally disrespectful. Asked for manager and she wasn't there. Got number to call from Jackie (receptionist). Jennifer (not sure if title) said she only had one more day there anyway. Called Alea (manager) and told her not happy with way we were greeted. Sorry was all we got. Price for exam with same insurance also doubled since last visit. This will be last one for us. Will go where our business is more appreciated. Should have known not to come back to Monarch after bad experiences in past. Nothing has changed.
service wait time
I had a scheduled appointment for what I thought was a cleaning at 2pm. I arrived at 1:39 pm. I waited in waiting area for 40 minutes. I had x-rays, then went back to exam area. I waited tgere for and hour and twenty minutes before someone came to give exam of gums. That is an insanely long wait time. Then, I waited another 10 minutes for dentist to see me to recommend something. Okay. So, I asked to get my cleaning done today possibly...and waited...again...for something I thought was already taking place.
If there is a wait time that significant, the techs should advise people before they waste an entire day.

Took my son there for a exam I was quoted 29 dollars for the exam and new patient X-rays well that was a lie, they charge me almost 440 dollars this was at the duncanville Tx location... the dentist was very in nasty with my son and kept saying how strong a toungue he had and to always remember that ... she was so out of line with him and although he is 22 he was so upset about it... and then she filled his two teeth one of which was infected? Wth is going on there? Where is the other dentist that I have been going to fir years?Very disgusting behavior by the dentist and very inapriopriate by the female dentist...this is not ok
In any way... but please tell me why she filled his very infected tooth, now he won't go back there fir the follow up...
dentures
I got new dentures in 2015. Paid my account monthly on time. Went back several times complaint the they made my mouth burn unlike my old dentures. Was told new dentures made from different material. I need my dentures so I can wear them. They need to be made with a material I can tolerate. Please help me or refund my money and I'll go elsewhere.
