REFERENCE: Short – Term Disability - Appeal
Evidence of Insurability (EOI) is a health questionnaire that helps your insurance carrier determine whether you qualify for new coverage. This commonly requires answering a few simple health questions to determine if you meet health standards for requested insurance. An underwriter reviews the application to make a final approval for that coverage.
The EOI Form for NEW York Life Group Insurance Company was completed timely and sent to ICP.
There was no denial of coverage by New York Life – Apparently this was missed by your compliance, Audit or Underwriters. The employee birth date is clearly stated on the EOI. This should have been flag and common sense would have alerted the staff of NYL that ICP (International Cosmetics and Perfumes) employee did not qualify because of age and ability to collect social security! NYL accepted all payment premiums from the month April through the later of October 2023.
If my concern is not addressed in an appropriate time-frame (November 15th) I will be seeking legal advice.
