Unexpected debt from a medical provider appeared on my credit report for an ER visit
I have suddenly had a collection item from a medical provider appear on my credit report stating I owe a balance from an emergency room visit over 5 years ago. I have never received anything from this collection agency and this charge was disputed directly with the hospital, 5 years ago because it was not billed correctly through my insurance. The date of the visit was 2/18, and I went to the ER after slipping on ice due to winter weather, concerned I may have broken my wrist. My insurance, which I've had for 10 years, covers all emergency room visits from accidents at 100% with no copay or deductible. Since my visit was accident-related, had it been billed correctly, I should have owed nothing. I received a bill from the hospital for $352 for the visit. I disputed this upon receipt and never got a response, nor did it affect my credit until now, over 5 years later, with a collection agency claiming it's a valid debt. This is incorrect as the hospital's initial billing to my insurance was flawed. Further investigation into the hospital revealed numerous complaints of similar billing issues.

They have furnished this alleged debt on my consumer report and have failed to validate the alleged claim pursuant to Sec. 809(b)of the Fair Debt Collection Practices Act and according 609(a)(1)(A), you are required by federal law to verify, through the physical verification of the original signed consumer contract, any and all accounts that you post on a credit report. I demand this item be removed from my credit report immediately. I received one letter from Paramount Recovery dated May 21 But the actual date I received it was on May 28. I wrote a reply letter to them on June 19 and mailed it the next day. I have replied within 30 days after receiving their letter to me. However, when I checked my credit report on June 25, I found to my shock that Paramount Recovery had reported 3 negative items at the same time and caused a devastating 60 point drop in my Equifax score. Of the three negative items that were reported by Paramount, I did not receive one single written notification at all. The one letter I received from them was of a totally different alleged charge with a different creditor than the one that showed up on the 3 negative items of varying alleged amounts. In all of these cases, I have never heard of any of these creditors. Paramount is in clear violation of the Fair Credit Reporting Act by falsely and prematurely reporting to the credit agency without responding to me and giving me a validation of the alleged debts when I had replied to them within the proper timeframe. They sent me one letter with the creditor Cascade Capital Funding with one alleged debt amount, then went ahead and reported 3 items with an entirely different creditor Tennessee Emergency with 3 entirely different alleged amounts straight to all 3 the credit bureaus (Equifax, TransUnion, Experian) without notifying me in writing at all. They are in clear and egregious violation of my basic rights as a consumer and what they did was clearly illegal. Not only did they not inform me they were going to post negative items on my credit reports, they failed to provide me with any validation of alleged debts from these 2 different creditors. Please inform them they need to clear up the mess they created by removing the 3 negative items from my ALL 3 of my credit reports so my credit score can be restore. Thank you.

On July 8, I settled a medical debt with Online Information Systems. The amount was $50.79. After payment, it was removed from my credit report. On July 26, the same debt was reported to Equifax by Paramount Recovery Systems. I informed them the debt was cleared with another agency. Paramount requested payment proof and said upon receipt, the debt would be removed. They noted they acquired the debt in June . Now, they await Cascade Capital Funding's instruction to remove the collection account. I've contacted the original agency, but they've received nothing from Paramount. I'm getting no clear answers from either party. Repeated calls to Paramount, requesting a supervisor, result in being asked to leave a voicemail with promises of a callback within 24 hours. Despite leaving three voicemails, I've received no response. I'm trying to resolve a debt I don't owe. I've never been contacted by Paramount regarding this debt; all previous communications were from Online Information Systems. I learned of Paramount's involvement when Equifax notified me of the addition to my credit report.

I called the customer service department to inquire about this debt and the dates I was provided doesnt match any of my families health records. Paramount has placed four collection accounts on my credit bureaus. I've sent them letters asking for verification and they refuse to respond. I was not made aware of any of these bills until it showed up on my credit bureaus. They have sent me no information requested to verify the validity of the accounts in question. The only thing that I've been able to find out via the credit report is supposedly medical bills. Again, I've asked repeatedly for verification and validation of the accounts. They have refused to provide me such information. My next option will be filing a lawsuit for illegal collection practices. Trying to get this resolved, without any cooperation from them.

It was reported on June 16, 2022 but it says opened November 21. One in the the amount of $402 account number *** the second collection in the amount of $228 account number ***. The rest of the account number is filled with an x. Original creditor *** They dont even have the correct phone number of mines. Then I call to speak with someone she says that the original creditor is from *** of *** Bay which is also not correctly put on my report. She goes on to say that I visited the emergency in *** in 2017. She doesnt even have my correct number that Ive had for the past 7 years connecting to my business of 6+ years. Ive filed identity theft before on this issue in the past because Ive never been to *** in my life. I live in *** and have never had a reason to go to *** to go to an emergency. Then she proceeds to say oh someone used your ID to get seen at the emergency how would someone use my ID? Whats my ID number? This is information she does not have. Dear Complaintsboard.com, On Wednesday, July 8 I received unknown communication from Paramount Recovery Systems, L.P. *** was dated for June 22, which means it was generated, held in their possession and then mailed out 16 days later. This delay for this unknown debt stated that a response was needed within 30 days of receiving the notice to dispute such unknown debt. The late delivery of this communication doesn't legally allow for the legal validation of a debt allowed to a consumer. The deadline would be July, 22, 2020 from the time the letter was generated on June 22, to respond to such unknown debt, which is 9 days away from today. I do not even reside in the same city or state as Paramount Recovery Systems, L.P. The debt, creditor and the account listed is not one that I am responsible for and is invalid. The communication listed a company by the name of Cascade Capital Funding, INC. I have no financial responsibility to Cascade Capital Funding, INC neither do I with Paramount Recovery Systems, L.P., and I owe no such debt to neither and none of my records reflect the account number that was notated. There are no dates of service, no dates of any transactions, no description of any service on the communication and Paramount Recovery Systems communication only listed an unknown creditor, whom I do not know and an account number that is unknown and an unknown dollar amount. The communication was also mailed from an unknown sender's business of *** on behalf of Paramount Recovery Systems, L.P., which when I looked it up was linked to Allied Interstate Review. Once again I have no idea who that business is either and have no obligation to them either. This unknown debt was reported on all three my credit reports and has dropped my score. This is a scam and what supports this, is lack of the legal right and time allowed for a consumer to dispute and validate a debt because of the communication being mailed out only 9 days from the deadline of the legal time frame of 30 days of receiving a notice, which presents a false attempt for Paramount Recovery Systems, L.P. to manipulate and pursue for an unknown debt without any validation.

I received a collection notice dated May 22,2020 from Paramount Recovery Systems as well as Phoenix Financial Services during the first week of
I received a collection notice dated May 22,2020 from Paramount Recovery Systems as well as Phoenix Financial Services during the first week of June. On June 9, I mailed letters to both companies. In the letter to paramount, for account number ***, who has Cascade Capital Funding, LLC as the creditor and a balance due of $254.15, I specifically stated that I have not nor had I ever had an account with this company and disputed the validity of the collection. In the letter to Phoenix Financial Services, for account number ***, who has Tennessee Emergency Physicians PLLC listed as the original creditor, service date 06/17, and balance due of $103.19, I clearly stated that I did not claim ownership of that account and knew nothing about it. In the letter, they offered to "settle" for $51.60. I stated in my letter that I would be willing to "settle" the account for 30% of the balance, $30.96, and send the funds in within 10 days of receipt of something in writing accepting the offer. On June 25,2020, I received an alert on my credit monitoring accounts, that Paramount Recovery Systems had placed two collection accounts on my credit. This was very outlandish seeing that the notice Paramount Sent was not for Tennessee Emergency Services, it was for Cascade Capital Funding, which I have never heard of nor conducted any business with. The only thing I can think of is they are scammers are both working interchangeably. One states the collection opened date as 05/22, with an amount of $150.00 and lists Tennessee Emergency Physicians as the original creditor. The other has collection opened date of 05/21, with an amount due of $103.00 and has a balance updated date of 05/01. When I have called the number listed, it always goes to an automated system. I never received a response from this company regarding these bills. This company is in direct violation of several various state collection laws. I am attempting to make every effort possible to resolve this matter amicably through the Complaintsboard.com without pursuing legal action so that a more simpler resolution can be made. Both of these collection accounts need to be removed immediately from Experian and other credit reporting bureaus these erroneous collections have been sent to along with written verification mailed to me of such. My son, who just turned 24, is still on my medical insurance
My son, who just turned 24, is still on my medical insurance. While applying for a credit card, he found out he has a debt to Paramount Recovery, a debt collector, for medical procedures performed on January 16 and February 20 at the Chelsea Clinic in Norwich, CT. My son was not aware of this because he never received a letter or phone call to try and make arrangements to pay this amount due. I am responsible for my son's medical expenses, I also was never informed. I called Paramount Recovery Systems Monday March 23, and spoke to a very pleasant sounding individual, who explained these charges to me. I told her I would pay the charges in full ($877.41) using my Health Savings Account Visa debit card, and she said it is not the company's policy to accept this method of payment. I could pay by cash, money order, credit card, but not with a Health Savings Account Visa debit card. I explained this is the card I use for all medical payments, and she said it does not matter, this is the company's policy. I asked to speak to her supervisor, she transferred me to the supervisor's phone and I left a message on their voicemail. It was getting late in the day and I hadn't heard back, so I called again, this time speaking to someone different. I pretty much duplicated the conversation I had earlier with the first person I spoke with. This second person also transferred me to the supervisor's phone, and again I left a message. I noticed this supervisor was different than the first one because their voicemail message was different. Also, the first person I spoke to told me unpaid medical expenses go against the person's credit rating, which I found hard to believe, because I have been involved with MedConn and Arcadia collection agencies for medical expenses and they told me unpaid medical expenses do not go against the person's credit rating. I am skeptical of the type of business Paramount Recovery is. My method of payment for all medical expenses is with my Health Savings Account, but this is not allowed by them. I've never heard of such a practice. Today is Tuesday, March 24, and I still have not heard from them. My goal here is to 1) Confirm I actually owe this amount, that it is legitimate; 2) If the amount owed is correct, I'd like to pay it with my Health Savings Account; and 3) If this does impact one's credit rating, I would like to have this removed because there was never an attempt to ask for payment and this is impacting my son from obtaining a credit card. 7/7 @148pm MST I received a call (Rep. ***) in an attempt for medical debt collection from Paullino Assets (another debt collection firm) regarding 2 ER visits on 3/12 and 5/5 both in the same amount of $656 here in AZ. I requested the doctors names listed in the dates, and the rep said it was not listed. Normally the doctors names are listed in these types of debt collection recovery. I explained to the rep that at the time and currently I am still receiving state medical AHCCCS and that this billing was taken care, I was ready to provide the rep the number to the state funded insurance billing and was told they would not be able to verify the debt because of HIPPPA, I did explain that taking over the debt (that had already been taken care of by the state medical insurance) and contacting me was also a violation of HIPPPA because my medical information had been sold to them without my Release of Information. The rep stated I could provide a receipt showing that the Bill's had been paid in full. I explained that it would not be a receipt but a medical billing ledger from United Healthcare My Community Plan-AHCCCS and I could email it after I contacted MYUHC and AHCCCS, as they will need to open their archives as this was over 7 years old. The rep said a receipt would need to be provided. There are no receipts when it comes to billing and patients have state funded medical insurance, there are also no copays. The state funded medical insurance plans cover everything in emergency room visits. I tried to to explain that to the rep, who provided me the email address, in addition the amount for both dates were exactly the same. It is not possible when billing in emergency rooms to have the exact amounts on different dates, as the treatments are not the same. I did apologize to the rep several times as I know that it is their expected job duties to collect a debt. But you cannot collect on something that was billed and taken care of by state funded medical insurance. I explained 3 years ago another company said the same thing when they contacted me and I let them know my insurance is still the same with AHCCCS they apologized and said they would remove my information. Noted to the credit companies this is not an outstanding bill/debt. In 2018 this was not on my credit report, after the call I checked my credit report and now it is. My credit has remained in good standing with only credit debt that I am working towards paying as I finish school, I have children on the spectrum and I work hence the state funded medical insurance. I tried to explain to your rep but they were not listening, I offered to even provide them my AHCCCS ID number

On October 1st, Paramount Recovery lists opening a collections account against me to the credit bureaus. This was reflected in my November credit statements per Credit Karma. I was unaware this company even existed until they damaged my credit and potentially caused irreparable harm with a former employer. (1) I was unaware of the collection status prior to it impacting my credit score, as Paramount Recovery did not use the address and phone number supplied on the original debt, which is still the correct and accurate way to contact me. Rather, they wrote a letter to my former employer, who assumed it was related to my father's estate. I had to have an embarrassing conversation with my former employer, and may have ruined my opportunity to use that long term job as a reference as a result of Paramount's conduct. -I believe this violates both the required notification prior to reporting the debt, as well as contacting an employer. (2) This debt was already paid in full to a previous collection company. I have supplied a letter of satisfaction from the previous company to Paramount twice by email, which I was told was the only way to send it. They did not supply a fax number nor address to me. I have not received any acknowledgement of the email and have called numerous times to resolve this. I am always told there is nobody available that can help me. In fact, on 12/15, I was told there was a long wait of people for the supervisor and was discouraged from waiting on hold. However, the agent I spoke with then told me she just checked and the supervisor wasn't at the desk, so I'd have to leave a message. This indicated to me that it is the standard procedure to lie to consumers regarding supervisors. The agents have informed me they are unable to help in any way except if I wanted to negotiate a payment. -I believe this borders very close to extortion. Essentially, they have made it impossible to resolve this in any way except for offering them a payment. There are no channels for resolution, no acknowledgement of communication, just a demand for money. (3) In the course of attempting to resolve this debt, a Paramount agent took the issue even further by saying that they had another debt for me as well. This other debt they attempted to collect is passed the statute of limitations. I was given the sense that it was an offer that they would seize attempting to collect the debt I already paid if I paid a debt that was uncollectable. Essentially, again, the idea was that if I simply gave them money, it would go away. (4) After being unable to resolve this in a reasonable manner, I attempted to dispute this with the credit bureaus. On my most recent call, the agent told me she could see I had disputed it, and said that she wasn't sure how she could deal with it since I had filed a dispute. Given the continuing situation with Paramount, I took this to mean that they were further going to drag this out to punish me for filing the dispute. I assume it will be the same for filing this complaint. -I believe that it is unethical, and potentially illegal for a debt collector to punish me for taking actions to which I'm provided under FDCA.

I'm depressed and suicidal after dealing with unauthorized use of my personal information. Which resulted in this unauthorized item on my credit report. I sent letters over 30 days ago and they failed to get back with me with sufficient validation. This account violates the fdcpa and must be deleted immediately because those are serious violations and I could be compensated for them.
