Claims not properly filed acct # [protected]
Hazelrig did not obtain Pre-Authorization for Radiation Treatments 01/20/2022, 01/25/2022, 01/27/2022. They filed only BC/BS for services even though I repeatedly gave them my Medicare Card. I called about a recent bill (included) only to be told that the time limit to file Medicare has been exceeded. Medicare cannot pay if the Billing Office does not file the claim. The bill is more than my deductible. There are multiple claims that have not been filed January 2022 - May 2023.
Desired outcome: Claims need to be filed through primary Blue Cross Blue Shield and then through Medicare

Have been attempting to get this collection notice off my Equifax credit report for a bill that have been paid. Amsher Collection SE 4524 Southlake Pkwy, Hoover, AL 35064. Have send copy off checks to the company and talk with someone (manage) in the billing department of the hospital and the item still on my credit report. This have been going on for over a year.

It was not. I finally had someone on April 20 tell me that I never sent in an application and when I informed her I did she put me on hold and found it. She refused to speak to me about how they lost it. My account with UAB was paid on April XX XXXX and UAB reported me on March 17, which should have never happened. The creditor put derogatory marks on my credit on May 31st. There should have been communication between UAB and Amshare. They are saying I owe around $300 but I should not even have an account with them and I do not owe UAB any money at all. I have UAB saying I no longer have an account with them to call Amshare and I have Amshare saying UAB has to contact them to tell them to leave me alone. They are at a standstill and are not cooperating at all to get this resolved.

Incorrect insurance was billed causing a greater balance and then sent to collections before correct insurance was billed. Another patient's insurance was mistakenly added to my account and billed. This caused a greater balance than what I should have owed. I called to correct the insurance and was told to wait for a new bill with correct insurance and patient responsibility. They then sent me to collections before my insurance could even process the claim. After my insurance processed, what I and my insurance paid satisfied the bill completely. I have called multiple times to verify this. I have been told my account would be pulled from collections and yet I still get daily calls even though my balance is fulfilled. I have asked to speak to a manager or supervisor and been told I would be contacted but no one has reached out. Since then, I have called and no notes were made on my account that stated I requested a call from a manager.
I am disputing some charges on 10/03 from the emergency room in Gardendale. Account number ***. I wanted to dispute this bill for 9145.49 emergency on 10/03. IV push 648.00 dollars and another one for 440.00 dollars and a CT Scan for 5000.00. I email Lisa Lewis to look over this matter and she did nothing. Lisa Lewis did not even acknowledge my email. This should be looked at and some of my money should be refunded. I will not use UAB again if this the way they do business.

My Son had surgery scheduled with Dr. Carl N for 3.5.21. AFTER we drove from Calera to UAB South, the surgery was cancelled. Dr. N reviewed an MRI from November 30 by another hospital and decided surgery would be malpractice as cryoablation wouldn't help and might worsen the condition. No procedure was done. I wasn't refunded $50 and accepted it. Now, a bill for an additional $22 arrived. Nurse Shirin, who scheduled the surgery without checking the MRI, said she can't resolve billing issues. Billing sent me back to Dr. N's office, citing no cancellation notes.

Problem Date: 09/17 Purchase Date (Payment Date): 11/07 Payment Amount: $315 Amount Said to be Outstanding: $20 difference in copay between old BCBS insurance (40) to new insurance (60) I changed jobs in May, moving from one BCBS insurance plan to another. After proactively updating all my insurance documentation immediately, UAB nonetheless repeatedly ran new transactions against old insurance. Because of the ensuing confusion, I waited until all had settled to call in early November and ask to pay everything outstanding and to confirm that all had been run against the new insurance. I submitted a payment on the spot over the phone for $315 and was told that that was everything that I owed and nothing more needed to be processed. I didn't receive any further mail or documentation indicating I owed anything for October or before. On 4/3, I received a credit monitoring alert from my financial management software indicating a derogatory mark on my credit from a collections company (Amsher). I called Amsher, genuinely thinking fraud may be at play because all bills were paid to my knowledge. Amsher informed me that an outstanding debt $20 from UAB had progressed to them in October and never been addressed. I pursued with them and then with UAB directly the removal of this from my credit and both of them said their respective policies would not allow it. Several things make this a failure in their processes and vendor management: 1) I did everything in my power to proactively address the job change and subsequent insurance change and cost implications throughout the year. 2) I was assured that all debts were paid. 3) I didn't receive interim communications from UAB or Amsher about this outstanding balance and had no way of knowing or addressing. I complain against UAB due to the filing against the wrong insurance previously and because Amsher is their vendor, but neither were handled professionally or fairly relative to me as a consumer. 4) I've been to dozens of appointments since then and paid any costs associates with those visits. Their systems did not indicate any outstanding balance either. 5) An update to "PAID" on a derogatory mark on my credit does not alter the impact to my creditworthiness. I am being unjustly punished as the consumer and patient and given no avenues to pursue recourse. Thanks for your consideration of this matter.
