Scheduled an appointment on the 22nd of December came in went through all the tests and was better or where I should be to give went back to the nurse went through my questions and answers and was told that they will need a permission from my doctor to give. I did it not knowing why and so called my doctor and said they still have not recieved any form so I called Bio life and gave them the number to the doctor which she had from looking it up in her directory. so they finally recieved it and sent it back in on the 27th so I stopped backed on the 28th and they sent me back to the same nurse who was still very rude and short and said yes I remember you and scan your finger. after I did that she said that now their doctor needs to review it and I said from all the information that I read before and up to this point nothing was said about age or getting permission. She made the comment that the doctor will be in Friday 31st and I will recieve an email if accepted, it is now the 3rd of January and have not heard anything. This is from the location in Ankeny and if I don`t hear back from you it`s time to make it right for everyone else that comes in that experience the same thing I did. Randall Dreckman
Desired outcome: I do expect a response and a solution
The complaint has been investigated and resolved to the customer's satisfaction.
