"I want to bring to your attention that I sustained an injury while at your store located in Mansfield, Texas (3018 Matlock Road, 76063) on Thursday, April 25th. I promptly completed the incident report and submitted a doctor's evaluation notice to the store manager. However, despite being informed that the district manager would contact me regarding this, I have not received any communication from them to date. I would appreciate it if you could look into this matter urgently and provide me with an update on the next steps to be taken."
