My wife was admitted in your ER and the doctor sent her home after giving IVs
She had to return the next day and was admitted to hospital
When she was admitted was late at night after sitting all day. The hospital admitted her with her shoes on in the bed
My wife asked for some ice chips and she was told by the nurse I can’t keep walking down here I am busy!
The hospital did not give my wife her medicine and they were told what to give.
The hospital did nit provide her food did to a liquid diet requirement
After she got better they continued the liquid diet

Crisis mental health
On 03/16/23 Francisco Reyes a patient with previous mental history went to crisis stating that he needs help and doesn't feel safe and was scared he could harm him self or others. Now in order to get help from crisis, patients have to claim to be suicidal as Mr Reyes was denied proper service. He was discharge without proper evaluation by a (student) doctor with no experience and referred to new horizon. Patients on crisis where begging for water as staff ignored their request while denying access to the outside area.
Desired outcome: Proper human health and mental health with dignity for patients

If you need an emergency, don't go to Jackson Hospital, they leave you in pain waiting for hours, and you'll spend the whole day there waiting, it's a real nightmare. I made an appointment at 7:30 am, precisely to be the first to be seen, they put another patient in front of me, that is, my time is at 7:30 am and it's 9 am now and I still haven't been seen, this is a disregard for patients. This has happened to me countless times, unfortunately I have the Jackson card, so to pay less, I have to come to Jackson Hospital.

Emergency Department
I’ve been here since 9:15am with complaints of pelvic pain. It is now 11:58am and I’ve not been seen. A nurse even attempted to question me in another room with another patient and not respecting my privacy, which is a HIPPA violation. I asked the nurse in the ER to speak with charge nurse to make a complaint, I was brushed off and no one has come as of yet. This hospital has by far the worst service and I won’t bring anyone here.

The patient got out of surgery and was told by his nurse Faye that he could not have his belongings because where his belongings were kept were locked and that basically he had to stay without his glasses and his phone until the morning. She was very rude and extremely careless. The patient did not want to put his things away but was told by the morning nurse that he had to do so. When I tried to get help everyone was being rude and nobody cared. This is regarding a person that just got out of back surgery… He is in excruciating pain, he is beyond uncomfortable, and to top it off now he canyons see or have any contact with any family members after the surgery because his nurse Faye and most of the other nurses on call did not care. Thos is unacceptable and disgusting! I hope they treat Faye and her family members with this same care and regard. She should not be a nurse!

Patient jewelry missing
My sister Greisy D Herrera died on Jackson cvi b icu unit room 5 on October 20, 2021, they returned her clothes and phone, but not the jewelry she was wearing whe she enter to the hospital . I have been waiting fon an answer from guest service, and there is no answer, the jewelry still missing and i want an answer, this is very sad . She was cery sick and some was stilling from her, that is unethical, un moral, and very disappointing to the family. My sister jewelry was a diamond earrings and a diamond ring, and i wont let this go .

I like to start by saying this is my third spinal fusion surgery in 2 and a half years at Jackson Memorial Hospital with Dr. Gjolaj from University of Miami.

October 21, 2021, 3rd spinal fusion surgery
Oct. 21, 2021 7pm to Oct. 22, 2021 7am
After awaiting for several hours in the post opt department, I was brought to the 6th floor orthopedic surgery recovery. When the nurse came into the room, she told me I had to take an Oxycodone pill. I explained that I could not take Oxycodone because I was a recovering alcoholic as well as taking psychiatrist medication. The nurse became very angry with me. My boyfriend called the Doctor on call in order for him to give me Demarol and IV Tylenol the medication my doctor and I had previously discussed. Dr Gjolaj had left notes in my chart not to give me oxycodone. That same night the new nursing shift come, I was half asleep when the nurse said she had a medication for me. The nurse put the medication into my IV and left. About an hour later I started feeling very weird. The room was spinning and the things in the room where moving and getting longer and smaller. I pressed the call button, the nurse came in and told me that she had given me IV Valium. I asked the nurse if my psychiatrist medication interacted with Valium. She said that the pharmacy would have notice if there where any adverse interact and she left the room. I did not see a nurse until the following shirt a 7 am.
Oct. 22, 2021 7am
The new shirt nurse come in and told me she has my medication, Oxycodone and IV Valium. I explained to the nurse that I could not take Oxycodone and that the IV Valium give me a bad reaction. The nurse got really upset and told me that was Jackson's "Protocol" to give Oxycodone to EVERYONE after spinal surgery… I explained to her that this was my third spinal fusion surgery at Jackson Memorial and that I never took Oxycodone or Valium, because my Doctor and I had discuss my post opt treatment. She was not receptive to what I was saying. Her response was "I call your Doctor" and left the room.
At 9 am my boyfriend came to see me, I had not pain medication since the night before. My boyfriend when to the nursing station to see what was going to. The head nurse told him that I would take the Oxycodone pill. After my boyfriend started screaming at them, they called the Doctor on call. The on call Doctor told me that my Dr Gjolaj was in a out of town conference. I asked the on call Doctor to please read the notice in my file that states that I can't take Oxycodone. The on call Doctor read my file and said "oh yes, here it is, ok I'll tell the nurse to give you IV Tylenol and Demarol Injections.
This should have been the ended of the problem but it was not.
Oct. 22, 2021 to Oct. 23, 2021 (night shirt)
When the night shirt nurse came in with my IV Tylenol, she told me that I need a new IV line because it was clogging (I had 2 different IV done that day). The nurse said she didn't know who to put the IV because my vain where hard to get and that she would have another nurse come in and put a new IV. 2 hours later, I was in a lot
of pain, I press the button to call the nurse. A male nurse came in with a pill of Oxycodone so I explained that I need a new IV line in order to receive my Tylenol. He said to talk the Oxycodone or that I had to wait until my nurse came back from break. I asked for the Head Nurse, he told me he was the Head Nurse and walk out for the room. I got up from the bed by myself and walk to nurses station. At the time I was crying and in lots of pain. I was asking for help to the nurse at the station, when the male nurse came out of the office and said to the other nurse, "I told to talk the Oxycodone but she refuses, there is nothing we can do for her" and when back into his office.
Luckily, an asst. nurse felt bad and held me, walked me back to the bed. She sat with me and calmed me down, she also call my nurse on her cell phone who was on break. My nurse returned from break and gave me a Demarol injection. I was without pain medication that day for a total for 4 hours.
Oct. 23, 2021
The morning shift come in and a new nurse come into my room and again offer my a Oxycodone pill, I asked her to please read my file on the computer which states my medication. Luckily, she was nice and read the my file and gave me the correct medication. My boyfriend came to visit me at 9 am. Around 10 am, a nurse (not my nurse) came in and said I had to take the Oxycodone pill. My boyfriend got really upset and call Elena "Nursing Director" she came into my room, my boyfriend and I explained what had happened. Elena listen to us, then she said " I hope you feel better, thank you for the feedback" and walk out of the room.
I asked for the Social Worker but she never came.
At that point, I understood that no one would help me.

I was offered Oxycodone every 6 hours by different nurses.
When I told them that I am on another medication, they would get upset. My boyfriend was in the room from 9 am to 11 am/6 pm to 9 pm visiting hours and also witness the nurses become upset that I would not take Oxycodone.

My Doctor came to see me early in the morning he was really upset that the nurse where pushing Oxycodone during my stay..
I was release on Oct. 25.
During my pervious spinal fusion surgeries in 2019, I was NEVER asked to take Oxycodone. The nurse followed the Doctors order and I did not have any issues.
I do not know what Jackson's new "Protocol" is but they are creating Opioid Addicts by insisting that ALL the patient talk Oxycodone. The nurses disregard the patient pass/present additions or any psychiatrist medication they maybe on.
I truly believe that the orthopedic recovery unit on the 6th floor.
Is an Oxycodone pill mill.

I am a post kidney transplant patient, I am very thankful to Jackson Memorial for the outstanding service I received.
My complaint is with my Coordinator recently assigned to me Elizabeth A Pearl. The way she speaks to me is unacceptable,
her tone of voice is not mild but aggressive. She speaks fast and some times I have a problem understanding her, and she seem to get frustrated when I try to get clarity.
I always try to treat people with dignity and respect and I expect the same. This is not a one time Issue but an ongoing situation, I think it would be in our best interest to have someone different call me with test results and instructions.

I called to check on the status of my family member... I left my number three times within the last three days the doctor never called back.. On 8/31/21 at 10:10 pm I called and spoke to the nurse. The nurse on duty the night of 8/31/21 that is lanston christopher williams nurse for room 112 bed 1.. She informed me that she cant give certain information cause it has to be given by the doctor which I understand.. As I was explaining to her that i've been waiting for the doctor to call me and my mother for the pass three days she hangs the phone up in my face as I was trying to check on my loved ones condition... And see is there a way that I can leave an urgent message for the doctor to contact me or my mother to know some updates about his condition and test results that were taken while in the er on 08/29/21. Along with a possible day he would be discharged... I feel that it was unprofessional of a health care provider todo such a thing when someone is concerned about there loved one and is only trying to see if their family member is ok.. I feel that it was rude and disrespectful...

I called in and obviously said something to irritate Glenda in the scheduling department; she kept asking me for bits of information "what's your name, let me put you on hold" ten minutes would go by, then she'd get back on the phone ask "what's your address, let me out you in a brief hold" another ten minutes would go by. I was humiliated hearing the giggle in her voice and after 45 minutes of this had enough. I work in a scheduling department and there's no excuse for treatment like this- to be specific I was calling in to make an appointment with a high risk OB and explained that I was in a great deal of pain from endometriosis and PCOS and my referral said "to be seen within 7 days" she refused (with no good reason to schedule me within that time frame) which was upsetting but not as much as hearing her laugh (subtlety) at what she was doing taking tiny bits of information and making me hold. I've never had anyone take my name then make me wait ten minutes then take my address, then make me wait another ten minutes to take my insurance information, etc.
Someone might want to remind her that a little kindness goes a long way.

Quick emergency assistance
Im 74 years old, Im diabetic and I also had a short heart attack therefore I started having organs malfunctions like retaining fluids and also a strong gallbladder pain, one of the reasons I went to the Emergency JMH on August 9th, 2021 at 6:30 PM. I have Medicaid and I didnt receive a proper assistance. They forgot I was diabetic and didnt provide me insuline. I was there more than 12 hours waiting to be treated with no pain relief and I was in deadly pain seated. My family were outside making the impossible assisting me in everything they could but Natalie the receptionist started hanging up the phone on them. Every person my son in law talked to he was told the emergency room was full and there were a lot of patients to be assisted as well but none of them were in my shoes with deadly pain and all of them could walk to the bathroon, I couldnt do it myself bcs I was in a wheelchair. My son in law spoke with both shift supervisors but not results but all patients coming with the Rescue they went straight inside to be treated. I finally left the Emergency next morning because I could have die there if my Cholecystitis continued getting worse. My family took me home, I took a shower and they called the Rescue and in less than 20 minutes I was treated at Mont Sinai and today im going to have a my gallbladder drained. Just as a record Mont Sinai was full as well but emergency priorities are better driven there than JMH or they could just be more consicious that people waiting in line to get covid test results can wait and I couldnt due to the pain and dead risk.
Desired outcome: I like JMH, I would like the emergency room team on duty to be more priority assistance driven and understand that one day any of them could be in the same situation.
My Brother went to Jackson Hospital, Monday October 4th 2021 complaining of a headache he had this headache for a few days, test were done on him, a procedure was done, a camera was install to take pictures, after the test the doctor said they saw a blockage on his brain . Suspected something going on with his heart, and depending on their finding they would send him to AUB. The doctors stabilized him and sent him home, the blockage was no longer a blockage, but old scar tissue . He had to go back to the hospital after being released because he still had a headache. And a leaky incision, And he told the doctor he felt like something was holding him down . The doctor replied you can move . ( such a nasty bed side manner ) where was the human decency, the compassion, what about the oath you took as a doctor . This patient is 59 yrs old, he left the hospital numb on his left side, a swollen incision, a headache, and with a blood clot on the brain that the doctors clearly saw but ignored and sent him home in that shape shame on you . To know doctors, who took an oath, who suppose to take care of patients sent my brother home like in this condition . Saturday the 9th He went to Baptist hospital and they did emergency surgery to remove that blood clot shame on you !’ You saw that blood clot ! No human kindness or decency!

The undersigned is Argeo Dominguez on Tuesday, August 3, 2021, and my address is 13120 SW 242 Terrace in Homestead. I am filing a formal complaint with the Agency for Health Care Administration against a facility or clinic belonging to Jackson. Memorial Hospital known as .Jackson Memorial Mental Health Clinic at 15055 NW 27th Ave, Opa-locka, FL 33054 for the following reasons,
First: Incorrect procedure of the clinic regarding the treatment of the relatives of its patients and clients, the staff of this mental health clinic is poorly trained or does not apply the appropriate procedures to process and understand the complaints of relatives and does not listen or they do not want to listen or they do not order them to listen to those who are closest or relatives to their patients day by day In other words, the service to the public is terrible, and their Justification is that they cannot reveal patient information, when the family knows more information than the workers themselves.
Second. The Human Resources department of this Hospital has a preference for a specific racial group in this clinic especially, segregating the other groups. There is a notable racial imbalance in their personnel. knowledge towards some of the individuals who are not understood.
Third, Dr. Louis B Antoine, who treats the patient, does not deal well with family members either, and he is unable to provide the appropriate medication in the correct dose, and proof of this is the lack of stabilization of the patient, or his professional practice is inadequate. prescribing wrong doses.
Final conclusion. The quality of the medical service is very poor in this clinic. This clinic is not aware of its medical responsibility, and the civil and legal responsibility it has when exercising its functions and its employees are mediocre, poorly educated, poorly trained, and they do not have your job as like its main objective. Discriminating to the person Hispanic or Latino race, which is unconstitutional and illegal. No one monitors the conduct of these workers by phone or mail, or by surveys, or in any other way.

I was calling the Jackson memorial pharmacy phone number at 340pm on July 28th and I asked a lady on the phone if my son had his medicine there and she was yelling at me on the phone very rudely I think she hates her job. I tried calling back again she hung up the phone several times. I, Gwendolyn P. Williams is filing this complaint against Jackson Imaging center located at 1825 Park Place, Montgomery Al. 36106. On November 25, 2020, an unsupervised student technician, Brittany Jones performed my annual mammogram. The student technician compressed my right breast over an exceptional amount of time resulting in severe pain. As a result of the technician's negligence, I now suffer with severe tendonitis, tissue damage, lung spasm, rotator cuff tears to my superior labra, and supraspinatus tendon of my right shoulder that continue to require medication and therapy. The technician was told the image prior was painful but did not make any adjustments. While waiting in pure horror that any movement would cause more pain, I began to silently pray that she would quickly release my breast. She then in a low, panicky voice told me to hold my head up before releasing my breast. At this point, I was angry and I told her that it was too long and severely painful. She replied, "I had to make sure nothing was in there." Shocked at her response, I stated, "You are going to make something there." I left there in pain and not knowing the full extent of my injuries. I assumed hot showers, soaks, alcohol rubs, and/or medication would alleviate the pain.
On November 26, 2020, I completed a survey about my experience in which I responded it was painful from start to finish. I also stated the technician did not seem knowledgeable and was insensitive to my pain.
Due to the pain not subsiding after the Christmas holidays, I became worried. I also realized that I had not received the results of my mammogram. On December 30, 2020, I called the imaging center to inquire about it. I was told it was normal and would receive a copy in the mail. However, I did not. After not receiving anything in the mail, I called the imaging center on January 6, 2021 and was told no one in radiology was available. So, the receptionist said "maybe I could help". I told her that I had not received the results of my mammogram. She told me I am supposed to retrieve it from my doctor. After I stated to the receptionist that I never had to do this before, the phone was hung up.
On January 18, 2021, I walked into the imaging center and spoke with Stefanie Denim to request copies of my records. Ms. Denim then asked Aimee, a technician, to view the painful vein on the outside of my right breast. Aimee implied that my injuries were nothing. Once I disagreed with that, she became insulting. Later, Mrs. denim stated that my 2019 and 2020 mammogram images would need to be edited due to being too long and could be picked up on January 19, 2021.
On March 8, 2021, I left a message for the supervisor, Jim Straud but never received a return call.

Scheduling appointments
They put you on hold forever when you try to make an appointment and when you finally get to talk to someone they put you on hold again then the line goes dead. Then when you walk in the security doesn't let you get to registration to make the appointment in person. You can't tell when a patient is having an emergency by their physical appearance. Some people have very high threshold for pain.

Yesterday morning, my wife had a serious accident and was taken to the Ryder Trauma Center. I was able to be with her all the time, as everyone in the Center was extremely nice and accommodating. I was sent to the lobby only during her ct-scan for 30-40 min. I waited, however, more than two hours pacing up-and-down, as I was not only nervous, but have a hip problem, so I cannot sit for an extended period of time. The receptionist seemed to be irritated by this, as she called a guard who asked me to leave the lobby immediately and wait outside. He claimed it's Jackson's Covid policy. Although he let the only other visitor in the lobby to stay and it was clearly just a pretext, I complied and started walking around the building. The guard followed me and requested that I wait in front of the entrance. I told him that I can neither wait indefinitely, nor sit on the ground (there was no chair of bench), so I have no choice, but to leave for home. I asked him to please call a taxi, as in the rush to the Center with the ambulance, I left my cell phone home and he was the only person around I could ask. He just shrugged his shoulder that this is not his problem. This behavior is inhuman under any circumstances, but inexcusable at a hospital. I don't think this guard and the receptionist on duty around noon shouldn't be employed by Jackson.
Security
The security guards at this hospital are thoroughly rude and love to belittle people. At least half of them need to be terminated.
