Miami Dade EMS took me to the Jackson West Medical Center ER on February 22, 2025. While I am very pleased with some aspects of the medical care I received before being discharged on February 26, 2025, other aspects amount to negligence. My patient ID is Burkhart, Logan T -6009864.
1 – While in the ER, I was given IV antibiotics. The IV bag stopped flowing concurrent with my being transferred from the ER to a room. I was moved to a room; however, the ER personnel and the floor personnel did not communicate regarding restart of the antibiotic IV. After several hours, I realized the IV was not running and advised the nurse. Upon verifying the physician intended for the IV to be run until the bag was empty, the IV was restarted.
2 – Upon discharge, I was given prescriptions for six (6) medications. Those prescriptions were filled at the Jackson West Pharmacy. Obviously, I was not at my normal alertness after requiring days of hospitalization; thus, I did not check the bag of medications given to me by the pharmacist tech. When I got home, I opened the bag of medications and was disgusted to find only five (5) of the prescribed six medications. Equally important is that one of the five medications was Albuterol/Ipratropium; however, the pharmacy had failed to include a nebulizer or, alternatively, instructions telling me which nebulizer to purchase.
3 – Upon arrival at the ER, I provided a list of the medications prescribed by my Primary Care Physician and my Gastroenterologist. That list of medications included Esomeprazole Magnesium 40mg twice a day, 60 minutes before meals. One of the five Jackson West Medical Center prescriptions filled at discharge by the Jackson West Pharmacy was for Pantoprazole. The Jackson West Medical Center Gastroenterologist failed to discuss the addition of Pantoprazole to my regimen of Esomeprazole Magnesium. What are the risks and possible side effects of taking both medications concurrently?
4 – During my care at Jackson West Medical Center, the doctors decided to perform an upper GI endoscopy, esophagogastroduodenoscopy (EGD). During the EGD, tissue was taken for biopsy. I requested a copy of the result of the biopsy that I could provide to my Gastroenterologist. I was told the report would be sent to me a few days after discharge. Today’s date is March 9, 2025 and that report has not been received.
5 – The Jackson West Medical Center Gastroenterologist advised me to make an appointment to follow with her. I responded, telling her I do not live in the Miami area and that I receive excellent care from my Veterans’ Administration Medical Center Gastroenterologist. My VAMC Gastroenterologist is aware of my acid reflux, GERD, Barrett's esophagus, the current care is based on a June 2024 EGD and he would be very interested in the report of her EGD exam and the biopsy results.
6 – After discharge, I attempted to connect to the Jackson Health System at MyJacksonHealth.com to access the record of my hospitalization and the results of the biopsy. Upon entering the required fields, a Jackson Health System popup stated I am not eligible to interact with the Jackson Health System.
