Fraud?
My father-in-law (patient) for whom I purchased this policy had to visit urgent care because of fever and cold. The date of service was 06/02/2014. The urgent care directly billed HCCMIS. Around 10/25/14 (first communication) we received a mail in our letter box stating explanation of benefits with reason code 130 that the claim has been closed because we failed to submit requested documents to them. The reality is, we never received any documents from them and they don't have any evidence of proof of delivery etc. On 10/27/14 after talking with their customer service representative we submitted the claimant authorization form and requested them to reopen the case. Today, 12/1/2014 we received a letter stating that our appeal to consider this case is rejected because we failed to originally submit documents within 60 days after date of service which is 06/02/2014, but as mentioned above, we never received the documents and we had no clue what documents need to be sent, the agency from which we purchased this Insurance called InsuBuy.com said that we don't have to worry if the medical provider directly bills but that's not the case at all. Now the claims appeal is cancelled and looks like I have to pay for the medical expenses out of my pocket after spending hundreds of dollars in the comprehensive insurance premium, this is shocking and financially very troublesome for us, the entire family and we never expected to get cheated like this.

Every claim denied
My parents purchased this insurance 6 months ago. My mother needed to have some tests run after her health started to decline a few weeks ago. My father called HCC to confirm the CT scan and MRI were covered and was told yes. Now that the company was billed for the cost they are denying the claims. In fact, they have denied every claim we have submitted. I am in the process of filing an external appeal as well as engaging the SCC to investigate this. In the meantime, my mother desperately needs surgery, but we cannot get pre authorization from HCC to proceed. If my mother does not have this surgery (to remove pressure from her brain), it could be fatal. We don't have months to spend arguing with them. I cannot believe, having now read all the complaints about this company, that they would still be in operation and not shut down. I will take this to the news and social media to find resolution. The stress that these criminals are causing my elderly parents is outrageous.

Costs of hospital stay not covered ($5,000)
Last year, I'd to stay in hospital for 3 days during my holidays in Turkey (due to a lumbago). It dealt with an amount of about $5, 000 which HCC Medical Insurance should cover. But they didn't, and it seems that they are "playing" with me. They sent to me three different exclamations of benefits (of about 5, 000 - 7, 000 and 10, 000 Dollars) and with different reason codes why they won't cover my treatment. I'd a lot of e mails with such company and they let me wait several months before they finally pretended not to cover, because I didn't make my claim in time. I verified and I could prove that I made my claim within 60 days. When I wrote to them again, they only answered that I can follow my case in the login section of their web page. It seems that they don't want to cover at any rate - maybe also because Europe is far away from Indiana, consequently they don't fear any juridical consequences. Now I'm trying to find a lawyer in the USA who could resolve my case ...
I've just reported my problem to Indiana Department of Insurance and I hope they can help ...
I'm not US citizen and the problem is, that Eventus Plus Ltd (from London) sent me an invoice. They make a lot of pressure to finally settle the amount of 5, 000$ because HCC doesn't cover. That's why I need support from a lawyer!
You are US citizen? Forget it. They can't get a dime out of you. When they let you out of the country without paying, that was it; issue was all over. Forget it. (maybe HCC will get nailed, but it has nothing to do with you)
Feeling Cheated
Okay, I am writing this in so much frustration and disappointment. Somebody in HCC Medical has to answer this before I take the legal route.
I took my mother to Emergency Room as she complained Dizzyness early morning. I called in to pre-certify (twice) and I even made sure that the hospital knows how to use this insurance. My mother's only known pre-existing condition (that too very borderline) is Diabetes and I can produce supporting documents for her condition. How in the name of god, can her Dizzyness and Emergency room check can be classified as Pre-Existing condition. Somebody needs to explain that. I feel like I landed myself in a SCANDAL of Visitors International Insurance.
Not one, but all 4 Service code based claims were denied by HCC Medical insurance. This is ridiculous. So you are saying that an Emergency Room visit is not covered by your insurance eventhough you claim so clearly in your policy for emergency room visits. As I am not accepting this denial of insurance, I have asked HCC to reconsider this Even a non medical professional will laugh at the way this claim was processed.
During this entire period, I made sure that I call into your service center and check my claim status so that its not pending on my end. I read reviews in Visitors Coverage.com and bought this insurance now this bad experience is really making me frustrated and feeling CHEATED. .

Claim non-fulfillment
I bought Short term medical insurance from HCC. I went to urgent care in August 2012. The Hospital sent me the bill directly. Contacted HCC for claims. For two months, they didnt talk of anything. Later, when the hospital sent the same bill again, I asked HCC for not processing it. They told me that they didnt receive all the documents. I made sure the hospital / Primary Physician has sent all the documents. Then I called up HCC again in Jan 2013, and they accepted that they have received all the documents and would require 1 months to process it. From Feb 2013, every time I call HCC for the claim settlement, they say that they require another week. How many more weeks they need now ?
The hospital has contacted the debt collecting agency, and I have started receiving their mails for collection.
*** HCC looks like a scam company, and I am regretting taking its short term insurance.

Feel cheated from Atlas insurance
All the other reviews that you will find for Atlas service are either fake or from people who have not filed claim.. If you were to file a claim wth HCCMIS/ATlas.. They will respond to you after a year when the hosptital were to already send the matter to collection.. the whole turn around period has been around 1 year.. They denied the claim after 1 year of beating around the bush stating “ This condition is not covered due to Pre-existing limitation". It had no further communication what so ever.. Don’t even explain what pre-existing conditions are.. HCCMIS – Atlas program is really a horrible program from a hoorible company.. there s no point buying an insurance when you need it its not there even though you pay so much.. .. They will keep saying we have not received paper work from hospital only after a year they will say we have rejected the claim.. I will try to dispute charges with AMEX and see how it goes..

Medical Bill not paid
I got this Insurance for my mother from http://www.immihelp.com where ATLAS (HCC Medical Insurance Services) are listed as one one of the insurance company. I bought this Insurance, based on two reviews listed on the immihelp.com. Reviews were good, so got this Insurance for my mom. My mom's legs started paining all of sudden and couldn't stop paining with off the counter pain medication. So after two weeks we took her to doctor, and found that she has to undergo surgery. Doctors office checked with HCC medical insurance and HCC said it's all good to go and HCC will pay 80% for the surgery which is by the way according to Insurance plan I bought. Because 20% were from our pocket, we paid first 20% in doctor's office. After 2 months, we received the bill from doc office that HCC is not ready to pay bill for rest of the amount. When called HCC, they said the claim forms were not submitted. After a phone call with them, they said they are willing to consider the claims and will let us know about the decision. Within month, we got denial letter from HCC saying all the claims were rejected and patient is responsible to pay the amount of $28500 to doctors and this time the reason is pre-existing condition. I told them that legs did pain two year earliers but leg pain went away with off the counter medication. This time the pain just won't go away. I asked the doctor and he said that if this type of pain was there 2 years earlier, it would have needed surgery right then. So I gave example to HCC saying that if your head was paining 2 years earlier and it went away after taking off the counter medication and you again have head pain two years later and now the pain won't go away with off the counter medication and this time you discovered that you got brain tumor, so is this called pre-existing condition. HCC had no answer for this. I personally think HCC medical insurance from ATLAS is fraud. Please don't waste your money in buying travel insurance from this company. You might end up in Medical debt and mental stress for rest of your life.

I wouldn't recommend buying insurance from ATLAS (HCC Medical Insurance company) and the guy you see Muhammed Yasin pretending to help you is not actually doing anything. I now started to believe that its just company policy to post a sorry comment and pretend to help, but in reality do not do anything about it.

EBD53, I think you should follow up with them to see what paperwork is missing from where, and accordingly have them all the necessary paperwork. After that, they would have no reason to not pay your eligible claims.
My daughter bought this insurance and has had a numner of claims which were denied by the compan because they say she didn't provide necessary inof. We have copies of the inofrmation forms sent for each claim and the close the file without acknowledging receipt. The even do this when the doctors office has gotten a pre-approval for the procedure(MRI). They go out of their way to not pay claims and we are consuting with an attorney.
Please specify what the leg pain was about. Was it related to arthiritis or something else? Please provide the details.

I took Insurance from this company for my parents visiting from India( one pre-existing and one regular). My dad had a sudden pain in his tooth one day. We called up this insurance guys and told them our problem, they recommend us to go to the dentist immediately and to take care of the tooth. The dentist had to take out the tooth since it was and emergency and we could not wait until he goes back to India. We submitted the claim to HCCMIS but they rejected the claim saying it was not and accident. I called up the company but the call center guys can't give us any answers. We dont know if this was an accident but the guy we spoke with before going to the dentist confirmed that this claim is covered. But once we sent the claim to them they are refusing to pay us.
We took this insurance after checking the reviews and were planning to take it for my parents and in-laws for few more years. But now we have started looking for another company for my in-laws. the money we gave was useless. This is like donating money. We dont have to donate our hard earned money to these people, there are better organisations that we donate our money to. We want insurance coverage for emergencies. Dont care if its an accident.
please stay away from this company. The call-center guys refused to give me the direct contact to their supervisor, so no way to really talk to the higher management guys.

HCC Denied All Claims
I have had similar issues as the postings on this site. After seven months of weekly phone conversations and endless communications with HCC, countless requests for resending doctor reports, all claims for myself and my dependents have been denied with no hope of appeal (around 30k). The claims include check ups, blood work, doctors visits, hospital stays, surgery and treatments. It is like I did not have health insurance although I was paying a lot for it. I found the HCC representatives to be uninformed, rude, and in a few cases lied to me directly on the phone. Note, I did receive my benefits package 7 months after getting a policy but 6 weeks after being canceled.
HCC I believe is the worst company I have ever dealt with after 7 months all claims have been denied and my policy canceled. Ingrid Robinson at ingrid.robinson2@yahoo.com has been helping me deal with HCC. Contact her if you are at a loss of what to do as well.

The worst inusurance company ever! My mom had this insurance for refugees. She never used but one day she was bleeding and she had pelvic pain and we went to the doctor. I'm 27 and my mom got a contraceptive when i was born 27 years ago, so that was in her for more than 27 years and it was the cause of the pain. The doctor told her to put it out and she ask him to check first if the insurance covered that. This insurance for refugees is supposed to be full cover and with no deductible. After the procedure the insurance refusee to pay it. The doctor even sent them a letter to explain how necessary was this procedure. But they said that they don't cover procedures for pregnancy. My mom is 50 and she was in pain. She was not trying to get pregnant. Now the hospital is asking her to pay an expensive bill, and she have been in this country for less than a year. I'm still calling to the customer services of HCC but they are mean, like really mean all the time. I got always the same representative Julio or Cesar and they never have anwsers. Now Julio told that they still checking the case, but they probably won't cover the procedure. They got the letter and the claim from the doctor till October. They did not do ANYTHING about it. Then I got a letter from the hospital trying to send my mom to collection. I call the insurance and they told me to that they had a letter from the a doctor with a claim what they don't have a formal appeal from my mom.. REALLY! I fill the appeal form and I sent it to them. ANYWAY I'm really upset about the way if this company to work with costumers. In this country sadly the health care is something shameful, but health insurances companies like this one just make the things more disgraceful. Now Im trying to get a decent heath care for my and my parents with the new Health Care of the government. But i won't recommend ANYONE to
pay any cent for being covered for this HCC company. They are just insensitive people trying to get money out of people like refugees, students, visitors... People only complain when their claims get denied simply because the buyer doesn't read their policy. All you have to do is read it before you buy it and you won't be disappointed.
This insurance is a scam - they will take your money, when something actually happens don't expect to be reimbursed, first they will tell you it takes 4 months to process your claim, then after almost a year with numerous phone calls and emails, they will not even respond to any communication - thieves, liars
As it is clearly written in the brochure, description of coverage and in the master policy wording, tooth pain or extraction etc. are not covered expenses until it is due to an accident. Please understand that medical and dental insurance are two separate things. If you wanted tooth covered, you should have purchased dental insurance or at least denal discount plan, as medical insurance does not cover that. In case of injury to teeth in an accident, it is considered medical problem and not dental, just like red eye or injury to eye is a medical problem but glasses and lenses are separate and not part of medical insurance. Please note that every plan is not meant to cover everything you need. e.g., does your car insurance cover oil change or flat tire? No. It does not mean the car insurance is any bad. It is just not covered.

i got the email from Muhammad Yasin, but its the same answer that they cant help us. they say they take customer service very seriously. But looking at this complain board it looks otherwise. we pay such a big amount for uncertainties and when need arises they have thousands of excuses and rules and conveniently refuses to pay. Luckily we had a small need but they still didnt bother to pay. Really bad customer service. Actually they yell and scream at you when we tried to reason with them. Please dont take insurance from this company because there is no use. Take it from a company where they will actually pay you when needed.

Claims Non-fulfillment and Scam
I bought Atlas insurance for my dad last May. My dad fell ill with fever and stomach ache during september and had to see the doctor. The doctor reccommended a scan and the bills were sent to HCCMS for claim and they came back to us asking for additional information and we provided the same. since then, i hear no information other than the claim is in process. i have called repeatedly to receive no information other than that the claims are in process and all of a sudden, they are telling me today that mostly the insurance would be denied as the time as expired. i was extremely shocked and i am planning to go to my attorney on this. I do not want any of my fellow folks to be cheated & scammed by this company ever. i also plan on writing about this in detail in my blog and bringing awareness on what this company does. KEEP AWAY FROM THIS COMPANY!
I would like to know whether the original poster contacted HCCMIS and what the resolution was. Whenever you post a complaint, please follow up here so that people know what happened. Most likely, the complaint got resolved favorly to the complaintant and therefore he is not bothered anymore to let others know. Unfortunately, people post only as long as they need help. After that, they go away.

Total scam + rip off. Do not use this company. I purchased a six month plan for my husband and I while we were in between major medical insurance coverage. It was $178 per month - auto debit only. I used Eheathinsurance to obtain our policy. In the HCCMIS plan brochure, they encourage you to go to an urgent care center if you are ill and not the hospital. It is only a $50 co-pay if you use an UC facility. Or so it states. They will charge you an additional $300 on top of your ER visit if you are not admitted and use the ER instead. Nice. I was ill with a chest cold that turned into bronchitis in July 2010. I went to a reputable UC center in my area and paid my $50. I keep getting bills from that Urgent Care for $141.00 still as of today's date. HCCMIS also wouldn't pay for a baseline mammogram as they claim to in their agreement brochure. That ended up costing me another $150. I have argued with them 'till I am blue in the face. I have now taken my complaints to the California Insurance Board and other government boards in hopes of a resolution. HCCMIS have the most stupid, inept and rude (snarky) people to deal with any time I call or email. I disputed my monthly payment to them in October and the will not even answer my letters or my bank in any manner. Save yourself time & money and get a better short term plan. I would dare say ANY OTHER COMPANY would be a better place to be then with HCCMIS.
I'm so glad you had a nice experience. This is why this forum is so well needed. Good and services are always subjective. I'm very careful with any plans I pick and had never had any trouble with any other company with "basics" being covered. While my resolution with HCCMIS went smoothly, it was only after I made my original post above that the company finally contacted me to work things out. There are a lot of other well run temp plans out there that do not make it as elusive and difficult as HCCMIS does for it's plan holders. Perhaps mine was an unfortunate, isolated incident, but I'm still skeptical...

The most terrible company I ever seen before, they are thiefs and liars. Never buy their insurance. They paid 20% for my bill! 20%! Their responsibility according to my insurance plan at least 80%! I'm trying to get explanation but they stalling for time, I can't get access to my account for 7 days already, I can't get access to my claims, I can't get access to anything, support doesn't tell me any information, it is just kind of nightmare, and it looks like it never ends. NEVER NEVER NEVER by insurance here!
Had the same experience with this fraudulent company. My son started having seizures after I started their short term plan. To make a long story short they keep on sending denials saying that they need more records. My wife has sent everything. Meanwhile UC Davis wants their money...$ 26000..This company is a joke. It's like they tell their reps to deny, deny, deny until you give up. Not gonna happen HCC...Stand the #%^* by.

I had typical case of urgent help, we visited doctor with a child after flu on Saturday, and received an answer that we need to cover everything on our own.
Any contacts with customer service just provide a links on a website, nobody is able to explain the huge discrepancy between brochure and reality or explain a cancellation terms before you submit request on their website.
Mailing exchange always starts from the same stage, like there is no previous contacts.
I think it's better to choose maybe more expensive service, but in a place where you can find customer service. Not here, unfortunately.
Very true, all the information provided by the individuals are true. HCC is total crap. I was skeptical before that the fraud could be isolated issue until I personally faced. The positive reviews about HCC are TOTAL FALSE. The HCC just do not care, they try to deny the claims in any manner by harassing for documents, online information and even children sickness also they claim also as pre-existing condition. The HCC is good if you never fall sick or visit doctor and proudly tell IRS that you have health insurance, but you should be ready throw 500+ dollars to trash every month. If you fall sick just keep few thousands in your pocket before you visit doctor. Advise you all, Please DO NOT MAKE THIS MISTAKE of getting HCC insurance.
DO NOT buy this insurance. My name was sold to them through the ACA website when we didn't qualify for insurance through ACA. It sounded just what we needed, an insurance to cover a major illness or injury. However they have denied every claim, but only after the run around of we need you life history medical records. All item were sent multiple times from multiple Physician offices. They will state pre-existing, or non covered. THEY will give you the run around. I am reporting them the insurance commissioner and would love to join in a class action lawsuit. DO NOT BUY!
How do I cancel their short term insurance? I got it from eHealth as my Kaiser policy wouldn't become active for a month. Now the Kaiser policy is active and neither eHealth or the notices sent to me that they are about to bill me for the next month have any useful links at all (or any on the notices) for cancelling this plan. Help please if anyone knows.
This is a SCAM! I have insurance with this company since 2014. I have it for family of 3. I went to see doctor for my wife in 2014, then for myself in 2015 and for my 2 year old in 2015. My wife recently felt sick and I went to see doctor. I was sent back from doctors offices because my past payments are due and none of the expenses paid by the insurance. Now, I keep getting calls from doctors offices. I tried to make multiple calls to the HCCMIS, but they are rude and do not help to resolve the issue. I forgot to mention that they changed my policy on their own in Aug 2014, and removed other 2 members of my family off the insurance and during that time I took my wife to Urgent care. Now they are denying to pay that.
I am having a very very difficult time dealing with this company. I have used an ER for my miscarriage. When the hospital sent them the claim, HCC sent the hospital and myself an EOB stating the discounts. The hospital said they cannot apply these discounts since they are not in network to the provider. I am geting continous calls from the hospital to pay the bill or else they will send it to collection. It is clearly stated in the STM contract I signed with HCC that any pregnancy related issues will be covered only the pre-natal visits will not be covered. This insurance is so messed up. Their customer reps don't know jack about what they are talking...
The HCC short term medical insurance is a joke! They claim you will get the Coventry discount for using them, that is a lie! Thank God I only have one payment left with them then it is bye, bye for good! They make you sign online documents then they deny you anyway! All I wanted was my coventry discount for a doctor I went to that accepts Coventry Insurance! This was for a non exsisting condition for a 188 dollars and they still did not let me have my Coventry discount! They are blood eating, money hungry people! Stay away as far as you can!

Even if you visit a provider in the PPO network and they bill directly, you still have to fill a claim form as claim form is not only for reimbursement but also for other purposes such as privacy release authorization, medical records collection etc. It is described clearly on our web site under Guide --> In Network Claims and that is what each representative in Insubuy would answer.
