The first email from them when questioning a charge on my Flex spending threatens to lock my account in 30 days. Why Well that comes later in the letter. You basically have no idea what they want initially. If you use your spending card to pay a doctor at the hospital you still have to send them the name of the hospital, address, and the date of payment separately because they can not figure out that they paid a hospital by the transaction. The site is hard to navigate. The sales rep is good at selling but man what a pain to try to use. I had their Accident Insurance too but dropped that after repeated headaches from trying to get a claim paid. Reminds me of the *** Novel where the company denied every claim hoping people would get frustrated and not try to fight to get their money...

Don't waste your money! I have been paying into an Accident Policy for a year. I filed a claim, which was denied saying it had been cancelled 7, yet I am still paying for the policy? I also set up an HSA through this same company, although I have contributed for nearly a year, only a portion of what has been contributed has been available for medical/dental needs.

I had AFLAC for many years. I switched jobs, they only will deduct from American Fidelity. What a waste of money! 4 years of paying premiums the claims Ive filed have been legitimate accidents.. all have been denied for various BS reasons (late date, tooth had previous work, etc etc) Im not asking for a million dollars here.. just asking that you actually pay out emergency services as you claim you do Once open enrollment comes, Im OUT!

American Fidelity Assurance Company The WORST, UNETHICAL company ever!

The WORST, UNETHICAL company ever! Their motto is "WE DENY EVERY SINGLE CLAIM, NO REASON NEEDED." If you are one day or 30 days outside of the "plan year", DENIED. We have no control of the timing for hospital/provider billing and medical insurance remuneration, to when the provider bills us. They simply lock your card so you can not use your funds! I can not unlock my card until I make a payment with my debit card to them for a *** I used my card for outside of the plan year, even though I have money in my account, a tactic to keep my money. Their requirements are incumbent on the customer which is another tactic for them to keep my money. The company denies every single claim hoping people would get frustrated and not try to fight to get their money, another tactic for them to keep my money. STAY AWAY FROM AMERICAN FIDELITY ASSURANCE COMPANY!

Of all the financial institutions I have worked with, the ABSOLUTE WORST on communication and customer service.They are lucky that they do not have to comply with normal standards, otherwise they would have been forced out of business years ago.

I dont suggest anyone using this company for disability insurance. They say 5-7 business days for claims to be processed. Mine has been over two weeks and when I call they tell me 1-2 more days. Ive been on the phone with them and thats pointless, the workers dont even know what they are doing.

Filed my disability insurance claim on May 17th. Today is June 26th. They have denied my claims 3 times so far, saying that they need additional information. Right now, I had to have 7 forms that I know of completed. Im dependent on others to complete these forms, which I am at their mercy to complete and return within a timely manner. Each time I am given a new form to complete, I start the 5-7 day review period over. They also decided to review my claim. They asked for my medical records dating back to October. I didnt have a policy with them until October 1. I will not continue to do business with them once this claim is settled (if ever). They want you to pay premiums directly from your paycheck but dont want to pay claims.

American Fidelity Assurance Company I have had nothing but problems with their FSA

I have had nothing but problems with their FSA. All of my charges have been for legitimate medical expenses at a doctor/dentist/hospital, and I constantly get requests for more documentation and threats to freeze my account. The normal response is that it is required by ***. The requirement of this documentation is for the customer not for . The record keeping requirement is incumbent on the customer according to what I'm reading in *** regulations. My spouse has an FSA with a different company, and I also know of others, and none of those companies have required them to submit the documentation. American Fidelity needs to re-configure how they do business. I'll be happy to leave, but it's through my employer. I have taken the opportunity to cancel every other product that I previously bought from . Looking at the other reviews, I'm not the only one. "

Only giving one star because I have to. I enrolled in coverage at work and paid premium for over a year but when I filed a claim they denied it and refused to pay. Dont waste your money

They are very slow at paying their claims and when they do they do , they take a long time to put the payment in the bank If you have direct deposit and when you ask a rep they tell you it's pending due to the fact that they haven't received all of the information on your case even after the *** has faxed the right information they requested and then they say it wasn't the right date? And it was the right dates !... After I checked with my ***.on my disability insurance, they just don't want to pay out the disability insurance payments...

This is the worst insurance! Go with Aflac instead. Ive been putting money into this company for almost 10 years and never used it. I finally decide to have a baby and they did not uphold their word on what is stated in the documents. My benefits were reduced and guess what?! Ive been off from work since March and will not receive the crappy check until sometime in May. I CANT WAIT UNTIL OPEN ENROLLMENT TO SWITCH FROM AMERICAN FIDELITY TO AFLAC.Signed and unhappy customer

American Fidelity Assurance Company Ive been a customer since 2008

Ive been a customer since 2008. Having multiple policies with this company, I now find myself in a position to change companies. First, in 2016 my son almost amputated is thumb. I submitted a claim and was denied. Keep in mind that my entire family was SUPPOSED to be covered. Well, needless to say, Amercan Fidelity didnt mind taking my biweekly/monthly premiums but again denied my claim because they claimed my son aged out but still on my active policy. Now here we are 2022! Ive submitted multiple claims for various reasons just to wait for a very long time (weeks) for processing. Its beyond ridiculous. Whenever you call to inquire they give you every excusejust to have you wait even longer. I have reached out to Aflac. The greatest perk with Aflac is the elimination waiting period is 0-7 days depending on the claim. Way better than 14 days through American Fidelity. I hope this information helps you in choosing wisely for you and your families.

We tried to call every number we have and you are connected to an automated system which of course tells you that you can go online. However, you cannot change your bank account number on line! When you wait, you either get leave a call back number or you get transferred to another number and even though you can stay on the line for a representative, you get cut off! Terrible customer service.

I was locked out of my acct. and needed to have my password updated. I tried 4 times to get an email sent and I'm still locked out. We've had terrible service every time we've dealt with them.

Inconsistent and what I've been told every time I call.. Had so many issues it wasn't worth having I would have rather gone back to work during my chemotherapy and my radiation and suffered rather than to take The misinformation and being lied to on the phone. I paid for disability insurance and they told me I was covered until the age of 70 they've been telling me that until just today they told me they could no longer cover me and I am a cancer patient and am suffering from from the after effects and healing then several surgeries I still have to go... I didn't ask for this cancer and I didn't plan on using this insurance but it's actually a c*** show... Now soon my disability is going to be cut off and I am facing more surgeries and I have no money coming in because they lied to me and they told Me I was covered until the age of 70 for the last 2 years they have told me... It only makes my health decline the harassment and the red tape and the upset and the sleepless night they have caused me.. I am high risk at my cancer coming back and my health is declining because of the stress..

American Fidelity Assurance Company I have used American Fidelity for years for unreimbursed medical expenses through my employer

I have used American Fidelity for years for unreimbursed medical expenses through my employer. They generally have paid my reimbursements satisfactorily. Unfortunately, their website is completely confusing, and hard to use. Speaking with a representative has been my preferred route in the past. They are always friendly, and helpful. Now, you wait for as long as a half hour to speak to someone, who may give you inaccurate information, or may not be updated on the present terms of your employer's reimbursement policy, which has changed due to COVID. I had to wait on hold, as the rep tried to access information which apparently was not in the system. Not his fault. He tried. This info was relayed by another agent, and partially inaccurate, but no changes were evident in the system. I asked to speak with a supervisor, who also looked into it, as I waited. She promised to call back within an hour. After waiting 24 hours for her call back, I called AF, myself. To date, they have updated their system to reflect the changes which impact me, but have not paid my claims in full, as they should have.

American Fidelity Assurance Company Im writing this as I sit on hold

Im writing this as I sit on hold. Its been a half an hour so far. I sent in reimbursement for an ER visit. It was paid. Now they are saying it was outside of the plan year. My card is frozen now. What difference does it make? It was a covered hospital stay. Bills sometimes come a year after given care anyways. Its impossible to stay on top of the dates. Our plans roll over from one year to the next anyways. Im dropping EVERYTHING I have with them. Every year its a hastle to get anything covered. I send in documentation constantly on the app. Then I get contradicting letters saying I didnt provide documentation. God forbid I get sick and die. I wonder if my wife could even navigate the policies I have, cause you guys sure as heck cant. For the love of God, Im still on hold. Will somebody please call me on my cell at (six one eight, nine seven five, eight one seven five) so I can get this taken care of? And dont write me some long generic response to my complaint here. If you guys spent half as much time actually serving customers as you do writing responses to complaints here, you wouldnt be in this situation, would you?

American Fidelity Assurance Company I would not recommend this company to anyone as of now

I would not recommend this company to anyone as of now. Also, if this company does not resolve our issues with them, we will ACTIVELY discourage others from ever signing up with this company. They consistently give conflicting information regarding our Flex Spending Account. While most customer service representatives appear nice and friendly on the phone, they do not actually DO anything to resolve problems. We now have over $1200 sitting in our flex acct that they are not allowing us to access to pay medical & dental bills. I am beyond frustrated at this point with the actions (and lack thereof) of this company. They have consistently given us the run-around when it comes to accessing OUR OWN MONEY, claiming the need for more & more *** regulations"... This is a scam plain and simple. I am convinced that this is how they profit, by freezing customers accounts until the end of the year while only allowing $550 to rollover to the next year, but all-the-while still taking the money out of hard working people's paychecks. We have been with them many many years, and had problems off & on that require a lot of time and energy to resolve... but this will definitely be the final year we attempt an FSA with this company if this matter is not resolved immediately.

American Fidelity Assurance Company American Fidelity has the worst customer service

American Fidelity has the worst customer service. If I could give negative reviews I would. I had several exchanges with the Executive for my area. I called the number I found online to get someone else to talk to because the Executive and I did not seem to agree. That person from the online phone number directed me back to the Executive. I even directly asked the Executive for contact details of someone above her. I never received it. She finally offered a set time on a Monday afternoon for me to call her, and when I did, I got an answering machine. I then sent an email asking her to send whatever information she wished via email. No response. A few weeks later, a guilt trip email from her. Needless to say, I have dropped all insurances with them and would advise others to do the same. The issue they were all avoiding was my request to drop disability insurance. Bought it due to COVID concerns and a new job. Decided didn't need it at a March meeting. They wouldn't drop it, said not until September. Initially, the Executive agreed to drop it. Payments ceased. Why couldn't it have ceased in March? Never got my refund for payments from April to July. Amounts to $281.60. NEVER EVER do business with American Fidelity!

American Fidelity Assurance Company I work as an auto technician in a local garage

I work as an auto technician. Previously, I had bilateral carpal tunnel surgery covered under workers' compensation. I had colonial disability insurance, which I was told covered all injuries, but it didn't cover work-related ones. I then swore off disability insurance. However, in 2020, American Fidelity convinced me to get their disability insurance, assuring me it covered work injuries. After being hit by a lot attendant, workers' comp accepted my claim, but American Fidelity didn't, citing a work-injury exclusion. This was frustrating as I'd asked specifically about this. It's a lesson that insurance companies prioritize profit over support. I didn't read the fine print, which wasn't shown when I signed up at work. I've warned my colleagues about this misleading information. Always read the fine print and be skeptical of insurance promises.

American Fidelity Assurance Company American Fidelity is the administrator for my school district's Flexible Spending Account

American Fidelity is the administrator for my school district's Flexible Spending Account. Let me tell you, they do a HORRIBLE JOB at doing this. They simply can't get their system to handle my daughter's Orthodontist monthly expenses properly. Every single month, my daughter had visits with her dentist, and they in turn sent the expenses to American Fidelity for reimbursement. American Fidelity was responsible to reimburse the dentist office, because those were eligible dental expenses. So what happened? Every single month, American Fidelity declined the reimbursement expense stating that they needed a copy of the ORTHO contract, even though they had the contract in their system since the beginning of my daughter's treatment. This happened EVERY SINGLE MONTH! These reimbursement denials caused my dentist office to charge me LATE FEES. It took me almost a year, and a trillion emails back and forth to get them to finally reimburse me for these LATE FEE charges that were caused by their system errors. To make things worse, the American Fidelity representative told me that my unused funds would be transferring to the following year's account. Well, THEY DID NOT. After a trillion emails and calls, I finally got the supervisor to call me, to simply tell me that I was given the wrong information. This misinformation ended up costing me $250... which wiped out ALL SAVINGS I was supposed to enjoy by opening this 125 FSA account. I might as well not have opened the account. I am extremely disappointed... the FSA125 should have saved me money, but instead gave me a thousand headaches WITHOUT ANY realized tax savings. No school district should entrust them to administer their employees FSA125 plans.

On April 27th I got a broken foot after paying my insurance policy for about 11 months I decided to use it as it was my right, during the last three weeks it was a total back and forthcoming with agents, after one of the agents lied to me saying the company does not record conversations when I was letting her know she was being extremely mean, unprofessional and with lack of empathy towards customers, the same day I got a call from another agent saying my disability would pay for my month of May as it was three weeks back and forth asking documents to off set the payable amount. *** called me again during this week on June 30th saying my disability would not longer pay as my earning are above 80% when I tried to explain she only kept on saying this was a final decision and nothing could be done I asked her to double check on my policy as I was already speaking with other agents and she refused to do it. Fidelity kept asking documents from my employer, doctors and delaying the whole process for weeks as supposedly my income was higher than the 80% earning stipulation but this is a part of my policy. If the Disability Earnings plus the gross Disability Benefit exceeds 80% of Your Monthly Compensation, the Disability Payment will be reduced by the amount exceeding 80% of Your Monthly Compensation which was not disclosed by *** that day but after weeks of reading my policy and going back and forth with agents I understand it. Why would you hide a part of the policy when taking the decision to hold my benefits? I have completely documentation same that fidelity was requesting me for three weeks before The agent said my benefits would be sent, not even considering I let her know for weeks fidelity kept asking documents. Everything is recorded in my conversation which each agents within this company.

American Fidelity Assurance Company I have paid for a disability insurance policy with this company for 30 years

I have paid for a disability insurance policy with this company for 30 years. I have been diagnosed with PTSD and I have made my disability claim. It was decided on November 17th that my benefits would be reduced to $184 for the month of December. Last month, I received 1700. They knew on 11/17 that I would be getting 184, they failed to tell me until November 30th, the day before the deposit went to the bank. They had two weeks to notify me of this and they did not. I had no time to make arrangements, phone calls, or even try to prepare for such a drastic reduction. I have PTSD and I am currently without the medication I need to control that. I am physically sick, and I am seeing withdrawal symptoms that I've never seen before. I am scared. I don't know where my mind will take me in the next days and weeks. I'm afraid that something bad will happen because I can't control my mind without the medication. I have recently moved across the country and I don't have a support system in place. One employee suggested I contact my Dr and see if anything could be done. This question, asked by Teresa ***, only served to prove that she had not been listening to me because my dr is over 1000 miles away and I haven't seen a new dr yet. How's that for professionalism? Before the sun came up today, I am almost 1000 overdrawn and I haven't even received the 184 yet! I will have no further income for at least 3 weeks and when something is deposited, the bank will get all of it. I've just finished paying off the bankruptcy that my ex husband caused, by myself with no help at all from him. At one point, I was collecting rain water to flush toilets. I have finally clawed my way back into financial daylight and now, because of their incompetence and inability to send out a notification on the 17th, I fear I've been sent into a financial tailspin that will lead me to bankruptcy again. I'm scared. I'm afraid of becoming suicidal and I don't know what I'm going to do. I just hope I'm still around to receive the next payment, and I hope the next benefit won't be a death benefit. Dana Spain is the person responsible for my being in this situation. If I end up back in bankruptcy or, possibly, dead from suicide, she is the person my family WILL be contacting, or better yet, our %%%%%% good attorney will be contacting her. Is this what I have made payments for, for 30 years? Does this seem humane, fair, or responsible? Is this what I should expect after paying for this every month for 30 years? An exciting trip back to bankruptcy court. Good job ruining a faithful customer's life. ***! Be miserable.

Terrible company. Around 5 years ago they were ok on paying, not great but if you called or resubmitted paperwork with items circled then they paid or still denied. In recent years the sales reps are the same, they will sell you stuff but really have NO idea about claims, they know how to hook you. The reps who do claims seemed to be encouraged to not give a flying cow about paying claims or even making customers understand or want to help explain during phone calls why a claim was denied. They will find any little reason not to pay. Even with all this Covid happening they are worse with their rules and definitions to accommodate that fact that they do not want to pay (yes, I know I am being redundant about they "not paying". It is shameful. I have paid into this company for over 16 years and I have filed many claims. I break a tooth, I send them what was sent to dental insurance company by dentist. AF denied claim (needed a more thorough description). I sent a copy of what the dental company paid dentist and dentist description and claim still denied. I call and complain and I just don't understand what else they would want. I then get connected to a "higher up" to explain things. I still am not happy and I state I don't know why I still have yall as a insurance company. This witch says if you don't want to use our insurance that is fine and I will send you paperwork. I said that is not what I said, You really don't care if you lose a customer? She states " I will send you withdrawal paperwork." I said wow, and hung up. You know, she did send me a form to remove them as my insurance company. I guess I should have used it and put my money in an account. I had a major reaction to some bug bites and had an E-visit recently since doctor did not physically see patients, *** refused to pay. Several years back, husband had shoulder surgery torn tendon, I stated he had 1 tendon repaired, I included all receipts and photos and they would not pay the complete claim because I didn't include backing information that it was one and not two tendons repaired. I again informed them it was one tendon, no need to worry about paying for two but NO, still denied. This last claim I just submitted, I received 100 for a repair of a torn tendon ( they had already paid the initial incident but allow to submit more on the same claim if it turns into a drawn out medical accident claim). Doctors thought only a pulled muscle initially and the torn tendon wasnt discovered until after months and months of *** and then an MRI. Around 11 months after initial diagnosis I finally got scheduled for surgery to repair the torn tendon and debridement of the bottom of elbow. It took so long that the tendon and muscles wasted away and had to be repaired. Well, according to *** the Surgery is not covered. THIS IS HOW MUCH THEY VALUE THEIR CUSTOMERS. Shame shame shame, we pay you a lot of money out of our paychecks over 300 dollars and this is how you treat us?

I signed up with AF through my job in March , They came on site, and were very accommodating. There were a couple months when I lapse, but got back on track and they were good. There were a few times when my husband wasn't going to make it and I called them to make sure everything would be good. They told me no problem he is covered if anything would go wrong. In Feb 2021 my husband passes away. Thank GOD I did not have an elaborate funeral and I got him cremated. First they asked me for all of this medical information. I sent it in late Feb., the only thing was missing was the cause of death on the death certificate. I finally get the cause of death, but in speaking with the funeral home, They told me the insurance company said the claim wasn't payable because his death happen within in a two YEAR window. *** This was a 25k spousal rider that required no examination. This is *** me off, but actually scaring me be cause

Through our city we have flexible spending with this company. Its been a pain since we changed to this company. Now I have over $400 in my account that they will not let me use. They say my documentation from 11 months ago is not adequate enough for me to use my money in my account that is withdrawn from my paycheck. The documentation sent shows that it is for medical expenses. They say its not detailed enough. Well IMO the details of our medical visits is none of their business. Hippa will inform them of this as well. Hopefully my employer will switch companies soon, because this one is a pain to deal with.

American Fidelity Assurance Company I have been getting monthly money from American Fidelity for a disability since 2017

I have been getting monthly money from American Fidelity for a disability since 2017. Then on May 21 I didnt get it and I called to find out why. They said that they needed my original social security disability award letter. I accidentally sent them the wrong documents that had the monthly money that I get from social security disability. I sent them the correct documents. Over my time using them, they asked me to complete yearly paperwork about my social security disability money and I was incorrectly completing the paperwork and they never helped me. Now they have decided to stop paying me the money that were paying me for 16 years. They told me that the reasons are because I didnt fill out the yearly paperwork correctly and because there is a over payment that must be paid off. Its ridiculous because American Fidelity never helped understand that I was completing the paperwork wrong. I assumed that I was doing it right until now.

I sat down with an American Fidelity representative last year. I was completely honest with her and explained that I had many pre-existing conditions. She told me no worries. They have this all the time and they will still be able to help me. Not only did the company NOT help, but I am now in financial trouble because of the misleading claims. The representative lied about the Disability, and also the *** I was told ANY time I need to go to the doctor, I will "get $100." Well, that was a lie, too. I have called numerous times to American Fidelity and I receive a different answer each time I call. The pre-existing condition clause is so vague, that it is completely up to interpretation. I thought that American Fidelity, being offered through a school district, would be a legitimate company to work with. I am finding that I have paid thousands of dollars for nothing!

In August I signed up for two policies with American Fidelity which are disability and whole life insurance. I was told by my representative that both policies would be deducted from my paycheck from *** starting in October. Since then this has not happened. I've reached out to ***, the senior account manager, who is my rep several times via email and he is not helpful and does not follow through. I've called American Fidelity twice about this and I get nowhere. They tell me to wait 5-7 business days. I'm very frustrated at this point. Nobody is resolving this issue and I'm not covered right now which scares me. Please help.

American Fidelity Assurance Company I have a Cancer policy through American Fidelity and I have not really had any issues with it until recently

I have a Cancer policy through American Fidelity and I have not really had any issues with it until recently. I have had some basal cell places on my arms, hands and face and they have paid accordingly until the doctor's requested I had radiation on my face. The doctor's were the experts regarding what should be done regarding my basal cell that is on my face right on my eyelid. The doctor's said the best way to treat this would be radiation since it was so close to my eyelid on my left cheek. Surgery would have to have been preformed by the Mohrs and many reconstructive surgeries, that wasn't an option according to the doctor Radiation was the only way to treat this type of cancer and the location of the cancer. There is nothing in the policy that states radiation will not be paid if it's basel cell and if it is locate near the eye. How could they have reformed surgery on the eyelid? I want the amount I should have for the radiation reimbursed to me.

I closed out my annuity account. I mailed the documentation on May 27 and cannot get any response since the last contact via email on June 1 from *** advising notary is required. I responded indicating that the HR Director advised that the document printed out from his office would replace a notary because it contains signature from a rep from American Fidelity. I emailed on June 19 to see when I could expect my annuity check with no response. Six weeks later, I have heard nothing and have not received a check. I have saved all emails to and from so I do not have documents to upload at this time.

American Fidelity Assurance Company I signed up through my employer with American Fidelity Insurance

I signed up through my employer with American Fidelity Insurance. They never sent any insurance cards, policy numbers, or any way for me to utilize what I was paying for. On several occasions, I tried calling and emailing anyone to get them to send me cards or policy numbers so I could go to the doctor as I have a serious surgery that needs to be scheduled. I have never been contacted in return and its been almost a year. I have to walk around in pain everyday because I cant afford my surgery on my own. I cant schedule dental or eye appointments either for lack of policy/card numbers. I would greatly appreciate someone helping me out bc Ive paid $50+ every other week for almost a year and I feel like theyre stealing my money! I wouldve stayed with my old insurance company had I known I would be taken advantage of. Can someone please contact me and tell me what I can do? This company needs to give my money back and they should have their license revoked. Thank you.

American Fidelity Assurance Company On December 23 I purchased 2 policies from American Fidelity

On December 23 I purchased 2 policies from American Fidelity. The Hospital and Cancer policies. Both policies had an eligibility date of February 01. On January 6th I received an email confirming the policy and policy start date. I started to pay for these policies with the payroll period ending January 31. On the February 14 I was involved in an unfortunate fall resulting in lacerations to my face and lip, also head trauma that resulted in a trip to the emergency room. On March 03 I filed a claim with American Fidelity. This claim was then denied payment by American Fidelity stating that I did not have coverage until an effective date was not until April 01. Immediately I contacted my sales rep. She confirmed that my effective coverage date was indeed February 01. Through further investigation, I found out that my sales rep entered my information incorrectly. No one in the company seems to want to fix this. Phone calls, emails. no help.

American Fidelity Assurance Company I was taken off of work on [protected] by my PCP

I was taken off of work on [protected] by my PCP. I ensured all of my claim forms were completed prior to submitting my short-term disability claim to American Fidelity (AF) on 6/14 They were to process my claim in 5 to 7 buisness days and after 10 buisness days I decided to call and get it expedited since they had surpassed that time-frame. On 7/2 I recieved a call from the claim specialist not only denying my claim until they recieved medical records from my physician, but also abruptly questioning why I was taken off and the status of my treatment; I spoke to a supervisor about her demeanor too. My physician faxed the records back on 7/6 and today during my visit even explained that she spoke with the representative from AF whom also asked her clarifying questions about my disability. After inquiring for the umpteenth time today 7/12 they said they will need to again seek more information from my doctor and it will take another 5 buisness days prolonging my payment.

American Fidelity Assurance Company I was told when I was in the hospital that I had a heart attack by my doctor!

I was told when I was in the hospital that I had a heart attack by my doctor! I was admitted and had a procedure done while I was there. I called my HR dept. and got all the forms needed from them to apply for my critical care insurance! I was also sent the forms from American Fidelity. I brought them to my doctor's office, they filled them out and faxed them directly to American Fidelity I also mailed them the forms. Then I was asked to get the file from the hospital which I did. They turn faxed the forms directly to American Family also. Now I get from American requests for the same forms that have been completed to returned to them. How many times must we go through before they act on the matter? This is not the first time that I have been asked to do all this. Several years ago I went into the hospital and had the same problem with them. I fax and then mail the original. It looks to me that they do not want to honor the policy. They say the paper work is lost or that they never received it.

On 1/24, my ex-husband passed away, my son, ***, who is 15 is one of his heirs to the two life insurance policies that he had. The claim was assigned to ***. I was told to submit the claim forms in my name on behalf of my son. So I did so. Then I was told the claim forms would need to be in my son's name. So, I submitted the claim forms again in his name. Then I was told I would need to submit a *** of *** for my son showing I handled his financial affairs. Now during all of this, my local Clerk of Court and attorney told me, as the mother and only living parent, I had sole control and rights over my son and this claim on his behalf, and that the court system didn't need me to do anything to act on my son's behalf. At this point, *** then advised I needed to be appointed Guardian over my son. My attorney *** up the document and the Clerk of Court approved it, only because American Fidelity was asking for it, they again advised this was not

American Fidelity Assurance Company On July 2 i talked to Kleber *** to cancel my short term disability plan

On July 2 i talked to Kleber *** to cancel my short term disability plan. i was informed by him that if injured on the job I could either claim short term disability or workmans compinsation, but not both. i was told by Kleber when I purchase the plan that it worked like Aflac where whatever workmans comp paid , they would pay the differance. this was not true so I told him to canceil my policy. I took the workmans com. benifits which was twice as much as my short term disability. I just recently went back to work on light duty. when I got my check I noticed that American Fidelity was still taking money out of my check, so I called the agent Kleber to find out wh. He explained to me that I needed to sign a form on the computer. when I first talked to him in July he said it had been cancelled if not why did he not let me know it wasn't cancelled. after 4 months I contacted him not him contacting me and now they have approxmately $400.00. I am writing this to you you because I was wrongfully taken advantage of. I have tried to talk to him, but am getting nowhere. i hope you can help me out. thank you.

American Fidelity Assurance Company My life insurance policy was cancelled without my consent in October , policy details omitted

My life insurance policy was unexpectedly cancelled in October , which I discovered when calling to pay during the Covid-19 lockdown. Unable to work, I was still managing my policies on unemployment. I have two other policies with American Fidelity and kept up with payments, but my inquiries about the cancelled policy go unanswered. I can't pay for an inactive policy, now with American Fidelity's conservation department. Multiple supervisors, including Austin Smith who promised resolution by the new year, have been involved, but with no update, and it's now February .

I sat down with a representative for the company who broke down several of the options that I could choose from and how much everything would cost me in addition to my health insurance and how much would be taken out of my paycheck every two weeks. I was told by doing certain things like getting a check up or x-rays they would pay me for doing that. I have reached out to this company several times and I know for a fact four times by phone and twice by email and have not one single time received any communication from the company. Im also paying this company double what they told me my cost was every month and I have no way of knowing how to access what Im paying for. Because Im not able to get any relief from the company itself this is what I have to do. I am wasting money by doing this because I dont know how to access my benefits and nobody is helping me. I have waited several months for some type of response and I havent received a single one. If this is how they conduct business I dont want any part of it and really I feel I should get a refund for the amount that I have paid so far since Im not able to access my services and if not that, I also feel that because they lied to me about how much it would cost a month I should also be given a refund. I shouldve done my research on the company before I decided to throw away money to support them.

American Fidelity Assurance Company Both my husband and I have been policyholders/plan care flex customers for many years

Both my husband and I have been policyholders/plan care flex customers for many years. In the past few years our customer service concerns have kept increasing. The latest concern has been *** life insurance policy. She has called for the past four months trying to get the forms to surrender/cash out the policy to no avail. The latest call was last Thurs.(Feb. 17). The wait on hold continues to increase with each call, so a call back number was left. The recording said they would return the call before the end of that day. We are still waiting for a call. We were told the forms were mailed in November when the first call was made. On the calls after that we were told that they would mail the forms again and send them by email. We still haven't received them. We have gotten other communications from them by mail and email, so they have the correct contact information. Our other concern is our Flexible Spending Account. Up until the past few years we were able to get payment in a fairly decent amount of time. It takes weeks if we mail the claims now. But, last year I mailed three separate claims in separate envelopes. Two were paid after calls were made. The other was lost by them and had to be resubmitted. Calls were made about that claim, but there was no record of it. Each time we call we are on hold for an hour or more. We make notations of the calls with dates. Claims were submitted by phone but it was difficult to get all the information in a phone picture. I submitted the claims again, later, but they were denied. Another claim was denied last year that I called about. It was in the proper timeframe, so I have never understood why. Basically, they have no customer service anymore. We could not recommend them to anyone in good conscience.

American Fidelity Assurance Company I retired July 1 and started collecting on my annuity with AF

I retired July 1 and started collecting on my annuity with AF. I am to receive payments monthly for 15 years. The first payment began Oct. 1, 2020. This annuity and my Social Security check are all that I have to live on. Payments are supposed to be direct deposited into my checking account the first of each month. This was confirmed when speaking with Sherry at AF on Oct. 26, 2020. When the 1st is on a weekend, as it was this month, my payment should have been direct deposited Monday, Nov. 2nd. It was not. Neither was it deposited the 3rd, 4th, 5th, or 6th of November. I called AF on Thursday, Nov. 5, and was told that it would be processed the 18th of Nov. and sent out. That was not acceptable at all. I didn't have my notes in front of me at that time, so I called back Friday morning, Nov. 6. I was then told there was a "glitch" and they were trying to figure out why my payment wasn't made when it should have been. I don't care what caused the supposed "glitch". I just want, and desperately need, my annuity payment. I told AF that nothing less that it being direct deposited to my account that day was acceptable. I was told they probably couldn't (meaning wouldn't) do that. Later that afternoon Tamesha called me and said that she was overnighting my annuity payment to me Monday, Nov. 9. As of this writing, I still have not received November's payment. I have no recourse with them. I put money into this annuity for years and years and years and now I can't get AF to make my annuity payments, and make them on time. To me, they should have done whatever it took to get my past due annuity payment to me on Friday, Nov. 6. If that meant they hired a courier to bring it to me in Stillwater, that's what they should have done. All I've gotten from AF in this whole nightmare is a bunch of excuses and no actual service. I am EXTREMELY disappointed in American Fidelity and their lack of integrity in doing business.

American Fidelity Assurance Company 6/14 my pain doctor advised me to stop working

6/14 my pain doctor advised me to stop working . [April 21 I suddenly began having excruciating/debilitating/disabling pain in my legs. I dealt with it until 6-2021 when it became intolerable, and I couldn't drive]. It took [1.5] months to retain the proper disability forms from the company b/c they gave me the run around. My pain doctor concisely completed all proper forms finally provided by American Fidelity DBA Boston Mutual, he faxed them. Boston Mutual DBA American Fidelity said they never got them, so I personally emailed them. On [protected] I received a check for $560.00 for June 28 - August. I called American Fidelity DBA Boston Mutual multiple times leaving VM's about this token check. My disability policy was for $1800.00 per month. On [protected], I was able to speak to a CSR *** who stated the claim was set and I would be paid. I was not paid. I was paid a token $400 a month from there on out. 10/11 DiDi called me, left a VM. I called back. *** answered and she said DiDi needed all Dr. *** Records. A doctor I never saw, ever until April. She then interrogated me asking why I was on leave, what was wrong with me, why did I ever see *** and she stated they asked *** for records and now ask that I ask *** for the same records. *** is NOT treating me for my disability. I called *** immediately. They sent the records requested 5 times over 2.5 months. I was provided all the fax confims. Dr.*** staff said the company is lying. I proceeded to facetime and watch as they sent them again for the 6th time. I still have not been paid after being interrogated and providing all forms requested. I emailed and mailed a 6-page urgent letter to which it took 2 months for a response, only to have DiDi *** call me again asking the same exact questions "What doctors have you seen prior to January 15th" NONE! I cannot make this up. This is insanity. This disability company intentionally defrauding consum
