Medical insurance claim rejected
I, Mr.Raj Iruthaya Ambrose had purchased a LIC`s Health Plus Plan with profits on 13/02/2008 covering my family i.e. wife, son and daughter.Premium was Rs15000 to be paid yearly through ecs from my bank account at PMC.First payment was made by cheque no.916002 for 15000.But in the month of march 2008 they (LIC) deducted Rs15000 through ecs this was brought to notice of LIC agent Mrs.Veena Sandip Gawli and asked her to inform the branch but again in the month of april 2008 Rs15000 was deducted this time i informed the branch, the call was attended by LIC, DO Mrs.Dani who assured me this wont happen again and she would refund the excess deducted after contacting her office at Hyderabad.After that i made it a point to keep below Rs15000 in my account.After repeated complaints no action by the Kalyn Branch 92D.My wife suddenly had severe stomachache she visited her family doctor in August2008, again the stomach pain reoccurred after 15days she was refered to a Gyno.She after doing all the test diagonised it as cyst in ovary it was the start, after a months medication she adviced her to get it operated to avoid further problems as pain was coming and going.She was operated in November for Hysterectomy on nov.8th 2008.I had informed the agent Mrs.Veena and also told her i would take legal action if i was not refunded the excess Rs30, 000 deducted from my account only after that they returned the money through cheque no 119550.My wife was discharged on Nov.15th2008. Itook all the necessary papers to the kalyan branch they said they did not have the claims form as this Health Plus Scheme was new and they awaiting for the forms from Hyderabad.So i gave the papers to the Agent Mrs.Gawli, i regurarly kept in touch with her.She got the forms in Dec 2008.After all the formalities were completed the form was submitted.In january 2009 i received a letter M/S Paramount the TPA for LIC that the claims request form was submitted late.When i informed the agent and the branch office about it, they said as the delay was from their side they would request the TPA to consider it.Till today i.e.january 2010 nothing has taken place. Now i want them to refund the intial amount paid by me i.e. Rs15000 to me as soon as possible.Also to take necessary action on the incompetent Agent and the Manager of Kalyan Branch 92D.My policy No.is924353854.

I have invested in the year 1.11.2003. Every year LIC of India was crediting my current Account with State Bank of India, Thousand Lights Branch, a sum, of Rs.4876 on 1st November.However the interest due on 1.11.2009 has not yet been credited by LIC. Please take up the matter with LIC

Association for Lifestyle Reform Adds Fraud Investigation Division to Protect Its Members

Chino, CA - February 23, 2010 —The Association for Lifestyle Reform (ALR) recently added a Fraud Investigation Division for customers to contact if they believe they did not purchase what was originally promised by third-party marketers.
ALR is a membership-based organization that provides discounted services to its members, such as pre-paid legal services and discounts on vacations, auto rentals, and prescriptions. As a membership-based association, ALR is able to provide discounts on behalf of its members by obtaining group pricing, including limited supplemental insurance benefits.
ALR engages third-party marketing firms to sell memberships. The company carefully screens the marketing firms it works with and sets and provides guidelines when speaking with potential members; however, there have been instances where independent marketing firms deviated from ALR’s guidelines. It is ALR’s policy to ensure the security, privacy and satisfaction of its members. To that end, ALR has established a Fraud Investigation Division. The division will take customer complaints regarding the sales process and investigate the issues regarding the marketing firm involved.
“We are very excited about being able to offer our product to people, however, some members have experienced an inferior sales process, ” said Brad Wessler, CEO of Association for Lifestyle Reform. “Under no circumstances will we tolerate bad sales practices from the marketing firms we work with. Therefore, we set up a division specifically for this purpose and a toll-free number for our members to call.”

During signing of blank policy proposal in front of do, bm and agent it was told that:
A) the policy premium matures after 3 year and may be continued upto 10 year.
After receipt of the policy the following discrepancies were found: the 3 year or 10 year maturity is not written. as per the policy, premium is to be paid upto the year :2037 and the policy will mature at the year:2038 ie, the premium has to be paid upto 9 years after retirement and the maturity value will be received after 10 years after retirement as my dob also recorded in the policy is 11-01-1968. neither do or agent is responding to my telephone call. please help and by either returning my premium amount in full or send me the exact minimum premium to be paid duration with full maturity benefit to save my money

With due respect, it is submitted that, I had surrendered my above LIC Policy on 04/08/2009 in the branch office of LIC at SCO No. 8-9, Choti Baradari [Branch Sr.-173] Patiala {PB.}.I have taken the receipt [Acknowledgment] for it. According to the rules, my due payment is to be calculated as per NAV of Future Plus recorded on dated 05/08/2009, which is Rs. 20/02.
But the manager of the concerned branch has refused me to make the payment as per NAV recorded on 05/08/2009 due to {according to him} failure of their system for registration. It is not my fault. The customer has no role in the system of the branch. I am being compelled to beer the loss due to their system failure. There is no such rule in the Policy.
I have also submitted my written complaint to the concerned branch manager Sh. K.L. Gupta on 10/08/2009for not making me the due payment according to the recorded NAV on 05/08/09.
Sir, I am being harassed, and due to this I am in tension. I need the justice. So I request you to personally intervene in the case and provide me justice.

Proposal No. 3955 dt. 9.9.2009, Branch Code : 729, Plan & Term 901 26 26, Yearly mode premium of Rs. 7500/-. In my policy document nominee name workly mentioned as M. Yavana Kani it is actually M. Yavanan Rani. I have given letter to my agent to change this before 9 months. But till now i didn't get it clear this issue. Kindly do the needful.

My father(Late MurliDhar Kanodia) was LIC Policy holder (Policy no: [protected], [protected], [protected]).He expired in Dec'10, First Nominee was my mother but she expired 8 years ago.Now I(Mrs Lalita Agrawal) and my Sister-in-law(Mrs Anita Kanodia) are second nominee of this policies.
We have given request for relieving of money i.e-: 20, 000, 20, 000, 10, 000.But we are not getting any response regarding the same.We have given written complaint as well, but no steps have been taken in this regard till now.
I feel very bad about the way in which such organizations are handling the customers.

Today morning I got sms on my mobile that a payment has ben made at LIC for Rs. 7115 . Well, It is very much shocking to me as First of all how could ICICI make payment on my behalf WITHOUT MY CONSENT which was mandatory as per my understanding with ICICI when their people contacted me to offer such utility bill payments facility and they assured me that they would never make any payment without my consent .
Secondly the due date for my next LIC premium is 28th may 07 , not 23rd may, this is again very disturbing and loss to me as I have to bear all interest and charges whatever you will debit to me on this account .
3 rdly, upon my call to inquire about it your one Mr. Anand from Mumbai assured me that no such transaction has happened and I should ignore this sms and should remain worryfree BUT I was not satisfied and I called up again and this time one lady picked up and with careful handling she definitely confirmed that 'Yes ' one such transaction has taken place but then she said that systems are down so she was unable to answer me on the question of 'CONSENT' as how could it be made without my consent. AND then she advised me to call after half and hour.
I am absolutely at loss to understand about such negligence and careless handling of credit card account of one of your customer. I want my this complain to be noted with full seriousness and action to be taken 1st. to reverse the transaction for which I never consented, 2ndly. no charges should be debited to me for which I am not responsible and thirdly take disciplinary action against the erring staff of yours who has done this careless transaction without my knowledge and consent.
Please apprise me your action or else I will have to take appropriate legal steps to protect my rights and safeguard from such faulty actions of yours and to recover my amount which you made to lic without my consent and also for the compensation for all the harassment and mental torture which I am going through.

I have been depositing premium regularly against the above policy at Connaught Place Branch. But now a days computer is not getting other branches connectivity. Today on 12th Nov'08 I have gone to LIC, C P Branch to deposit the premium but the dealing person has written LAPSE on the back side of the cheque and asked me to go to Branch 123, at Model Town Branch, New Delhi. Please let me know the status of my Policy. What is the reason behind it.

I (Dr. Brij Kishor Gupta) had purchased a LIC`s Health Plus Plan in the last October, 2009 with risk covering my family i.e. wife, son and daughter and premium was Rs. 20, 000/- yearly from LIC office, Gjaraula, Dist. J. P. Nagar (Uttar Pradesh) under Meerut Division (North-Central Zone). My policy number is [protected].
I have got premium receipt copy of Rs. 20, 000/- on dated 06/10/2009. But till date I have not received the bond. I told many time to my agent (Smt. Alka Verma, Code-0040325G) but she didn’t done anything. As per my opinion she is unable to help me and solve the problem. Also I have complained to LIC office, Gjaraula regarding bond. I become tired to follow the same.
So you are requested to do needful action against my problem and send bond on my following address as soon as possible.
