On October 4, 2024, I turned into the local Social Security Office Form W-4V. Received No response. On December 17, 2024, I turned in another Form W-4V to the local office. To date no acknowledgement that they received the form and were taking action. On 1-8-2025, I called the 800 number and waited for 4 hours for someone to answer the phone but finally gave up at 6pm and hung up. I then called back at 6:45pm but still no one ever came to the phone.

Claimed loss: I will have to pay more taxes because you failed to take taxes out at my request

Desired outcome: I want someone to acknowledge my Form W-4V and take taxes out of my check
