I reserved a car at the Hertz/Thrifty office at Ramstein AFB in Germany. I did this through Expedia, from which I purchased an insurance policy. When I went in to pick up my car I was told that the insurance I purchased (TravelGard/AIG) was not accepted. When I offered my credit card I was told I needed a physical copy of the policy with my name and credit card number on it. This is ridiculous. I've rented cars before in Europe and never needed anything more than the credit card insurance. I was forced to purchase insurance for 23Euro/day plus VAT. It cost more for the insurance than the car rental. When it came time to pay, I used the VISA card the agent wouldn't accept earlier for insurance.
