CVS 1020 S Broad St in Target, Philadelphia, PA 19146 I reported Melissa Giordano, pharmd at CVS Health, CVS pharmacy, aka Melissa Rose Giordano, of Philadelphia, pa who is also a temple university school of pharmacy graduate, NPI [protected], pharmacy license number rp449257 as well as Eric S Tsu, pharmd NPI #[protected] and pharmacy license number rp441633 for their comments and behavior to the US dept of state (Pennsylvania) bureau of professional and occupational affairs on 2/18/2022. I also previously reported a technician at this location for eating a kind bar (a snack) while filling prescriptions in public view. That technician is still employed at this location. Melissa Rose Giordano refused to provide me with her last name after I requested it to file a complaint following her providing me with misinformation regarding my prescriptions. Eric S Tsu also refused to provide me with Melissa’s last name after I brought to both their attention that I was provided misinformation which impacts my disability. I then asked them both for their license numbers and/or NPI numbers. Melissa Giordano argued with me and walked away as she spoke to me explicitly stating “you’re not getting my last name.”
Both Melissa Rose Giordano and Eric S Tsu falsified information, telling me that pharmacists don’t have license numbers or NPI numbers as they don’t write prescriptions. That is a blatant lie, as evidenced by public record for their professional titles.
Melissa Rose Giordano repeated to me that she was not going to provide me with her last name multiple times in an antagonizing manner.
Thankfully I was able to get both their names and license numbers as it is public record.
I have multiple disabilities and a service animal and cannot travel long distances nor drive on a regular basis, and this is the closest CVS to my residence as well as the in network pharmacy for my insurance plan but I am now in fear of visiting this CVS pharmacy location after the hostility and blatant misinformation that I received on 2/18/22 from both pharmacists Melissa Giordano and Eric S Tsu.
In addition, since both Melissa Giordano and Eric S Tsu lied to me by saying they don't have license numbers, I do not feel safe having my prescriptions filled by either of them. I also fear their ability to access my personal pharmacy records.
