Notified of benefits owed to me from a 401K savngs plan from Dex Media. Was given a plan administrator and plan #[protected]) but no instruction on how to claim or even a phone number. This is in reference to Dexmedia asset management committe(Plan Administrator (220 west Airfield Dr DFW airport TX 75261 - can someone give me contact information for this please?

Claimed loss: $15,000 owed single lump sum payment (reported in 1995)

Desired outcome: $15,000 paid
