I contacted Humana to confirm and enroll my individual dental plan early 2023. Humana confirmed to me that Wanda Moreta was the only provider as an HMO provider that was a participant approved in my plan at the dental office American Family Dentistry - Germantown, 7709 Poplar Ave, Ste 212, Germantown, TN 38138. Other dentists at that location were not. Given that, I enrolled with this provider.

Upon making my initial appointment and at my appointment in the office on 4/20/23 I confirmed my insurance and provided them with my card. My insurance was confirmed. There was no indication that my dentist was a non-participating provider. Had I known this dentist was not participating in my plan I would have pursued another provider.

My service is being denied payment as not payable for non-participating provider when both Humana and dental office originally approved provided as a participating provider in my plan.

I believe I was mis-advised.

I am requesting a review of this dental claim.
