Ongoing Lumbar Spine/Neurological injury from fall February 1, 2025. 03-04-2025, office visit w/Umidjon Rahman, PA-C/Geisinger Neurology, (GHP Neurology, 3 W Olive St, Scr, PA) Orders MRI exam of neck, shoulders & lumbar cervical spine w/contrast for 04-07-2025, (GHP Radiology, 435 Scr/Carb Hgwy, Scr, PA) Does not order rx for contrast pre-medication, (patient allergic reaction to contrast on patient's chart).04-07-2025 patient turned away from GHP Radiology due to no pre-medication for scheduled MRI. MRI rescheduled for 04-11-2025.
Patient had sched appt w/Dr. Philip Lee, MD following day of MRI on 04-08-2025 which was cancelled and resch for 07-07-2025. Patient explained to GHP symptoms: tingling, limb pin/needles numbness, severe neck and shoulder acute pain, cold extremities ongoing daily since 02-01-2025. GHP stated patient put on waiting list for earlier appt. No acknowledgement of urgency of situation. Patient was told to go to the ER with any complaints. Patient will retain legal counsel for ongoing injury caused by Geisinger Health Systems.
