My name is Jane Logan.
My appointment at the above location was on May 23, 2018 at 9 AM. The confirmation # for this appointment is/was KEP62304.
The issue is with the phlebotomist - name unknown.
I requested that a butterfly needle be used because the BD vacutainer's draw is so strong as to puncture through the back of the vein causing me to bruise from mid forearm to mid upper arm. At this time, the phlebotomist stated that this issue is her "determination to make" regarding the type of needle to use. She also stated that they do not ever have/use any butterfly needles.
She then pulled 2 needles from the drawer and said she would use the 23 gauge for the draw despite my protests and warnings.
As a result, the back of the vein was punctured and the bruising as stated above occurred.
I am not only unhappy with the bruising but totally unsatisfied with the attitude and behavior of this phlebotomist and think Quest needs to take appropriate steps in this regard.

home blood draw
I have home blood draws set up for every Wednesday between 9am-1pm for my INR. Yesterday morning I didn't receive a reminder call from my usual phlebotomist so around 12:30pm I decided to give them a call to see if they are sending someone new & if they'll be runing late. The operator Barbara informed me she wasn't sure of the time & will have them give me a call with the time. I said thank you and let her know in the future if they'll be runing late I'm going to need a call. I was told by the operator I could always go get my draws done at the lab, I then informed Ms. Barbara I'm aware of the local labs & if my doctor didn't think home draws were necessary I would travel or have medical transportation to the lab, I then suggested they could just do their jobs and call the consumer if your going to be over and hour & a half late past the appointment window. Ms.Barbara then threaten my health care & home draws with false claims to my doctor of rudeness on my behalf, she did. Instead of understanding I'm paying for these services & as a consumer deserve respect, she decided to question the validity of my condition & services due to frustration associated with the inability to keep up with scheduled appointments. I was discriminated against and made to feel like crap, it's hard enough being so young & dealing with deminising mobility due to my vascular health. Having anyone stereotype you and assume your able because you look, sound & of young age is hurtful. I'm always reminding people such as Ms.Barbara I'm unable to do a lot even though I'm young. I shouldn't have to it's not quest employees business.
billing department
These people are the single worst medical billing office I have ever worked with-- and sadly, I have had a lot of opportunity to talk with different providers and their billing departments.
I received services from Quest in my doctor's office in the summer and fall of 2017, and the doctor's office inadvertently reversed the order of which ICD10 diagnostic codes should come first on the lab order form. This happened 7/2017, 8/2017, 9/2017 and 10/2017. This billing department managed to correct one claim, and then proceed to bill me thousands of dollars for all the rest of them, instead of correcting the error on the other claims. I have spoken with them on the phone, emailed them and both my insurance company and my doctor's office have contacted them repeatedly... for nearly FOUR MONTHS now.
Meanwhile Quest says, "Oh we will halt billing activity on this account until we resolve the billing dispute, " 5 separate times, to 4 different people. And every two weeks they send me more threatening notices saying they are sending me to collections.
This company has no customer service whatsoever. My advice to anyone who has a choice whether to deal with any Quest labs whose testing and billing goes through Baltimore, MD to stay as far away as possible. Additionally they are also balance billing against their agreement with MultiPlan/UHC. I have no respect left for this company.
quest lab employee driver complaint
I was at one of locations where your drivers stop by to pick up blood works for your lab at Lakeside Medical Associates on Rinaldi Blvd. As i was picking up a patient myself and as I was ready to pull out of the facility I would haveto go around the building through the back parking lot where Quest Lab drivers park for their pick up. It is a 5 mph parking zone and all of a sudden your employee reversed back from his parking at a alarming speed wherehe almost hit my vehicle with the patient in the back. Doing so he made me press hard on my break and he continuously continued his route at high speeds and exiting the lot he made a illegal left turn onto Rinaldi toward Laurel Canyon. I barely got the chance to take a picture of the vehicle on my phone at the intersection. Vehicle plate number was "43441F2" white Chevy small suv. On the back said number "48". I believe it's the vehicle ID#.
blood work for toddler
We needed blood work for our 4 year old son because of constipation issues. They attempted to draw blood from one arm but missed so they drew the blood from his other arm. If they weren't sure about their 1st attempt & the vein wasn't visible, they should had not stuck him until they were sure about it. It's bad enough that he was suffering/scared about one shot; he should not have had to go through it twice. As if that wasn't enough, then they waved the tube filled with his blood in his face to show that they were done as if that was supposed to make him feel better... Not professional at all. When it comes to any type of medical treatment/procedure, you don't want a rookie touching your child.

I had an insurance change at the beginning of the 2018 year through my employer and had NOT received any information as to where to go for labs at this point. I always went to QUEST with my prior insurance and it was in network. My new insurance had NO copays for any labs whatsoever so it should always be free. My doctor ordered my lab on a QUEST order sheet and at the QUEST center I was told my United insurance was accepted and in network. Well this information was wrong as Quest is not in network. I did not know this and had my labs done 2/20/2018 at Quest anyways. I found out later that I had gone to the wrong lab and called QUEST to see if they could cancel the labs I had just done 3 hours prior and the rep said no, but that I could call and ask for a one time forgiveness to wipe the bill clean since this happens all the time when people change from one insurance to another. Well, the bill came today and I called Quest billing as told to by a Quest representative on 2/20/18 and spoke with Erica Jones who was very rude and accused me of getting my labs done at Quest when I knew I should not of gone there. Umm, why would I go there purposely and wait 2 hours for lab work in their nasty dirty office where everyone smelled like cigarettes knowing I am going to have to pay out of pocket for it? I then spoke with a supervisor, Josh Stanley who was just as rude and unprofessional. He said I was told wrong info and they cannot ever reduce or zero out a bill and told me it was my fault and I should of read my handbook. What handbook? I never received a handbook from United. I am disgusted in their unprofessionalism and rude behavior. It is bad enough to have to be sick and get lab work every couple of months, but to have to deal with ignorant rude people at Quest makes it worse. I have enough medical bills and to have to pay for labs that are supposed to not cost me anything BECAUSE I WAS SENT TO THE WRONG PLACE is disgusting. This bill should be wiped clean for me as it was promised in February by the first billing rep at Quest that I spoke with. Date of incident 2/20/18. client invoice # [protected].
billing 11 times more than what was initially communicated
My wife had undergone for some blood work which was related to her very initial stage of pregnancy confirmation on 27 Jan 2018 but before giving the blood sample we confirmed about the cost of those test by providing our insurance details and we were told that these tests would cost maximum of $100 and if insurance does not cover at all then these $100 would be debited from my Credit card so my Credit card details were also taken. We also had been provided a details containing the estimated cost of $100.
After 1 Month, I observed that $100 was charged by Quest on my Credit Card and then in March 2018, we also received a bill of approximately $1100, mentioning remaining due close to $1000. This is now really shocking for us to understand how come they serve a bill of 11 times more than what was told us before test (and estimation for same was also provided). If it would have been informed to us before actual blood work, we would not have undergone tests at all for sure but sending a bill of 11 times of what communicated before test is a total fraud/cheat/wrong information.
At this stage, when my wife is going through pregnancy time, we both are going through mental depression due to this which may impact her pregnancy as well. So we really want someone to check on this case as soon as possible because we reached out to Quest Billing department and they just said that Lab person did not give you correct and complete information hence nothing can be done and I have to pay this bill but why we are suffering for that?

Quest does not take the insurance negotiated price into account when billing. They charge insurance a high rate, get paid by insurance and expect you to pay the difference. I paid the amount the insurance company said was my responsibility.
I have called Quest numerous times, my insurance company reissued paperwork to them but I am getting collection calls. I am told that this will be sent to credit bureau if I do not pay $944.78 for 5 lab blood tests taken at my daughters annual physical.
I have had many issues in the past with Quest. In 15 years with the same insurance, I have never paid another company more than $60 and that is for numerous tests. Usually my lab bills are under $10.
I have spent so much time on phone with Quest, BCBS and doctors because of code issues etc. In the past I just paid the $209 to $300 to avoid the hassle and my credit being ruined. I am not paying $944 do to incompetence.
courier driver road rage
On 3/30/2018 at approximately 4:30pm on VA route 50 westbound, the bald/white bearded man driving car #75335 with license plate VTB-9212 stopped in the road next to me and yelled through the window telling me to use my eyes and using his hands to point to his eyeballs. I had put on my signal to change lanes when he was in my blind spot but saw him when he pulled up more so did not even change lanes. He still decided to try and intimidate me by stopping next to my car twice to yell through the window. He gave me the middle finger with both of his hands and was clearly very agitated. He looked like a complete crazy person he was in such a fit of rage. I couldn't help but laugh at how ridiculous he looked and he proceeded to mock me with his fake laugh. I am a young woman and did nothing to warrant this disgusting behavior of road rage and attempted intimidation. I took photos of his vehicle to report him. Someone crazy like him should not be operating a vehicle for Quest Diagnostics because it makes the company look bad. This man should be fired.

We were driving from the i25 to Belleview Ave by the Denver Tech Center at 207pm I believe.
We were turning right from the off ramp to Belleview (of which we have our own lane and it was clear for me to turn)
And your horrible road rage driver with white Hair was in the lane next to us and decides he's just cominging in my lane.
He begins screaming and cussing at us asking "what the [censor] are you doing" "I have the [censor]ing right away"
My daughter was like what is wrong with this guy he can't just come to our lane like that, is he going to punch us?
I said no just ignore him. Then he gets behind me and rides my bumper and almost hits my car as we waitied at the light to go straight and he was going to go right. Before he does he stops, rolls down the window and begins cussing at us again. I said it was our lane, you can't just come over because you feel like it.
He started cussing again and I said [censor] you too leave us alone and drove off

On Tuesday aprox 16:50 this driver appeared to be very agitated and was beeping at a few drivers. He ended up behind me as I was getting into the highway in Campbell Ave in west haven. I looked in the rear view mirror and then witnessed him throw a small pizza box out his window. I let him pass me and took a photo of the vehicle to obtain the number on the car and plate.
blood lab work
Went in to quest diagnostics on colonial drive in Margate Florida for lab work ordered by my physician. Upon arrival I signed in on a sheet of paper as they did not have an automated sign in( that was no the issue) I sat there for 40 minutes in a visible location in front of the window, 3 employees passed by, never addressed or asked me if I was helped already. The office was closing at 3 pm that day, I got there at 1:30 so I was not pushing the time limit. The employees could be heard in the back office having conversations not work related and they simply did not care about getting their job done, I simply left. Quest needs to hire competent employees and follow up with training. That is a shame!

Muy mal servicio para a tender a mi hijo con autismo, la señora alvarado le hablaba al niño en un tino muy fuerte le explicabamos la condición del niño y ni aun asi le tubieron paciencia.. tambien atendio a otra persona antes q ami hijo y le hablaba igual.. al parecer esta persona no esta apta para trabajar con muchas personas y menos con niños.. deberian de decir q no aseptan niños con una condicion en especial para no pasar por esto.. gracias
very sarcastic and seem upset
ZULEKHA. Was very sarcastic and got very upset because the Dr did not send what kind of lab work was needed. In the past other lab technician has help me by calling the Dr and getting the info. We arrived at 10:25am fasting and right now it is 12.00pm and she told me almost yelling that it was my responsibility to bring what is neede . I have been coming here for more than 2 years and this is the first time I have a problem.
inconsistent & discriminatory billing practices
I had labs completed 5/5/17 as part of an annual physical. Invoice# [protected] & [protected]. There were tests coded incorrectly either by the physician's office or by Quest to Anthem insurance. After months of unsuccessful calls, I sent a letter dated 12/20/17 requesting the codes that were submitted from the physician's office and from Quest to Anthem for the 5/5/17 and 11/17/16 tests. I had the same tests 11/17/16 that were billed and covered correctly. In addition, my wife had a few of the same tests on 5/11/17 that were billed and covered correctly. Quest refused to provide this information because they are either too incompetent to provide it or are hiding their previous incompetence in the billing department and discriminatory practices in not covering the same tests that my wife had covered. I will not pay for something that was not only covered last year, but covered for my wife this year. I expect Quest to do their jobs and either fix the errors or provide the information that shows where this process went wrong.

January 15, 2018 went to have my lab tests done. From more than 3 people working last year, I found a machine where to register my info. The same person who look my info in the computer was the person taking my blood. Then I want to see or get my results, I have to register in the internet. I tried for 2 days and it keeps saying something like "it can not connect-wrong info".
The info is correct!, Try to talk to a "human being" on the phone. No such luck!
What kind of jobs are the "human beings" going to have in the near future?

unprofessional attitude and supar method of taking blood
My husband and I went to Quest Labs for our yearly blood and urine tests.
We had a very unpleasant experience. There was a young lab person who took the blood in room 2. She was nasty and left a mark on my arm. She said I'm taking the blood from left arm and didn't even ask which arm is better for me like in the past they did. Then she didn't even talk pleasantly but kept looking at her cell phone and told me just pee in the cup in the bathroom and fill tube to blue line. By the way she left a big red mark on my left arm which I never had after a blood test before.
My husband wasn't so lucky. The girl made 4 unsuccessful attempts to extract blood from my husband. He was getting dizzy. Finally she sent him to room 1 where a very nice experienced lady was able to get blood from him at first shot at it. He also has marks on his arm from her. We look like needle users. It is sad. This girl should be terminated. She has no sense of what she is doing and is very unprofessional.
physical abuse by quest diagnostics employee at sahara ave, las vegas nv
I had blood work done on 11/30/2017 in sahara ave, Las vegas branch. There was technician, her name is "FLORIDA" who drew my blood was so rude and poking in 4 places to get nerve. I requested her to use the butterfly, She said "I know, that's what am using". Finally she got it in one place, while drawing blood she took out the needle in the middle, she said blood got stuck and did poke at another place for rest of the tubes. Even though she got the nerve while drawing the blood she kept poking and turning the needle left and right. I yelled at her saying please STOP and I even cried. She kept doing that. As soon as she removed the needle nerve was bulging. She did not even say anything as courtesy. It was very painful and turned out black totally for couple of days. It's physical abuse. I said I will complain, she said OK here is my name was showing her badge.
erratic/aggressive driver
This morning (11/30/17, approximately 8:37am) I was driving on Ridge Pike in Pennsylvania, when the driver of vehicle #75993 with PA license plate KKH3611 sped up to cut me off while their lane ended. I checked my rear view mirror, and there were no vehicles behind me where she SHOULD have merged in. I had the right-away, and they cause me to brake/swerve into oncoming lane to avoid collision. I also had a 1 year old in my vehicle.

I went to quest lab on 11/20/17. there was one guyanese looking woman there. I don't remember her name but she was the only one working there that day at that center.. Quest Diagnostics Queens Flushing, 4161 kissena blvd at concourse level.
she had a very nasty attitude. she was asking me what I do for living? keep staring at me instead paying attention to blood tube while she was withdrawing my blood. she refused to remove the tube even though it was completely full of blood because she wanted me to answer what I do for living. when I asked her to pay attention to blood drawing. she replied, "don't tell me what to do." she had a very bad attitude. people like her should not be drawing blood.
